神经根型颈椎病复发因素的Logistic回归分析

摘　　要：目的评价神经根型颈椎病的复发因素对复发的影响。方法选择神经根型颈椎病患者210例,采用中央随机方法,分为试验组106例,对照组104例。试验组采用手法治疗,对照组采用牵引治疗,两组疗程均为2周。治疗结束后1个月、3个月、6个月、3年随访,由患者自己填写复发及相关因素的患者自身评价表。结果治疗结束后1个月3个月、6个月及3年随访时,复发率分别为4.88%、16.46%、26.82%、39.63%。对随访患者复发相关因素进行Logistic回归分析,显示长时间低头伏案工作、长时间电脑操作、长时间驾车、高枕、不良坐姿、颈肩受寒、手提重物、颈部外伤、近视、情绪紧张、缺乏体育锻炼为神经根型颈椎病复发的相关因素。结论长时间低头伏案工作、长时间电脑操作、颈肩受寒、手提重物、颈部外伤等因素对神经根型颈椎病的复发具有重要影响。Objective To evaluate the relevant factors of the recurrence of radicular cervical spondylosis（RCS）.Methods 210 cases of RCS were divided randomly into two groups：a trial group（106 cases）and a control group（104 cases）.In trial group,manipulation therapy was adopted.In control group,traction therapy was used.The duration of treatment was 2 weeks in two groups.The follow-up visit was performed in 1,3 and 6 months and in 3 years after treatment separately.The patients themselves filled up the self-evaluation form of the recurrence and relevant factors.Results The recurrence rates were 4.88%,16.46%,26.82% and 39.63% in 1,3 and 6 months and 3 years follow-up visit separately.Logistic regression analysis was used to analyze the relevant factors of recurrence.It was shown that the relevant factors of the recurrence of RCS included working at the desk,long-term computer operation,long-term driving,sleeping on high pillow,bad posture,cold invasion to the neck and shoulder,carrying heavy things,traumatic injury in the neck,myopia,emotional stress and lack of physical exercise.Conclusion Working at the desk,long-term computer operation,cold invasion to the neck and shoulder,carrying heavy things,traumatic injury in the neck and other factors seriously impact the recurrence of radicular cervical spondylosis.
