二藤通痹合剂联合正清风痛宁缓释片治疗类风湿关节炎30例临床研究

摘　　要：目的观察二藤通痹合剂联合正清风痛宁缓释片治疗活动期类风湿性关节炎(RA)的临床疗效和安全性。方法将88例RA患者随机分为治疗组30例、对照A组30例、对照B组28例。治疗组给予二藤通痹合剂联合正清风痛宁缓释片治疗,对照A组单用二藤通痹合剂治疗,对照B组给予正清风痛宁缓释片治疗,共治疗3个月,观察患者28处关节疾病活动积分(DAS-28)变化,并观察肿胀关节数(SJC)、压痛关节数(TJC)、整体健康状态(GH)、血沉(ESR)等指标的变化。记录治疗过程中出现的不良反应。结果治疗组总有效率为86.7%,对照A组为76.7%,对照B组为67.9%。治疗组总有效率与对照A组、对照B组比较差异均有统计学意义(P<0.05)。3组患者治疗后SJC、TJC、GH均有改善,DAS-28积分降低,ESR明显下降,与治疗前比较差异有统计学意义(P<0.01)。治疗后治疗组与对照A组、对照B组SJC、TJC、GH、ESR及DAS-28积分比较差异均有统计学意义(P<0.05或P<0.01);对照A组和对照B组除在改善患者GH及DAS-28积分方面差异无统计学意义(P>0.05)外,在减少SJC、TJC及降低ESR方面对照A组与对照B组比较差异有统计学意义(P<0.05)。3组患者不良反应无明显差异。结论二藤通痹合剂和正清风痛宁缓释片联合应用的疗效优于单种药物,且能有效控制活动期RA患者的病情,安全性良好,可以作为中药联合用药模式应用于临床。Objective To observe the clinical efficacy and safety of Erteng Tongbi Mixture（Mixture of spatholobus stem and Chinese flowering crabapple for relieving stagnation syndrome of qi and blood）plus Zhengqing Fengtongning Sustained Released Tablet in the treatment of rheumatoid arthritis（RA）.Methods Totally 88 RA patients were randomized into treatment group of 30 cases,control group A of 30 cases,and control group B of 28 cases.The treatment group was administered Erteng Tongbi Mixture plus Zhengqing Fengtongning Sustained Released Tablet,the control group A was administered Erteng Tongbi Mixture only,and the control group B was administered Zhengqing Fengtongning Sustained Released Tablet only.The treatment lasted 3 months in all.The changes in disease activity scores（DAS-28）,swelling joints count（SJC）,tender joints count（TJC）,general health state（GH）,and erythrocyte sedimentation rate（ESR）of the patients were observed and the untoward effects in the process of treatment were recorded.Results The total effective rate of the treatment group was 86.7%,that of the control group A was 76.7%,and that of the control group B was 67.9%.There was statistical difference in the comparison of the treatment group with the control groups A or B（P0.05）.After treatment,the SJC,TJC,and GH of the three groups were all improved,the DAS-28 scores lowered,and the ESR decreased significantly,the difference was statistically significant（P0.01）.There was also statistical difference in the comparison of SJC,TJC,GH,ESR,and DAS-28 scores of the treatment group with those of the control groups A and B（P0.05or P0.01）.There was no statistical difference in the improvement in GH and DAS-28 scores between the control group A and B（P0.05）,while in SJC and TJC reduction and ESR decrease,there was statistical difference in the comparison of the control group A with B（P0.05）.There was no statistical difference in side effect in the three groups.Conclusion The clinical efficacy of Erteng Tongbi Mixture an
