加味补肝汤对糖尿病周围神经病变患者神经生长因子的影响

摘　　要：目的观察加味补肝汤对糖尿病周围神经病变(DPN)患者的临床疗效,并探讨其神经保护作用机制。方法将80例DPN患者随机分为治疗组和对照组各40例,另选40例健康体检者作为正常组。治疗组和对照组患者均予以基础治疗,治疗组加服加味补肝汤,对照组肌肉注射维生素B1、维生素B12,两组疗程均为8周。于治疗前后检测患者血清血管内皮生长因子(VEGF)、胰岛素样生长因子-1(IGF-1)水平,同时进行肌电图神经传导速度(NCV)的测定并观察临床疗效。结果治疗组总有效率为82.5%,对照组总有效率为57.5%,治疗组总有效率显著高于对照组(P<0.05);两组治疗后NCV均明显高于治疗前(P<0.05);与对照组治疗后比较,治疗组NCV的升高更为明显(P<0.05);两组治疗后均较治疗前血清IGF-1水平升高、VEGF水平降低(P<0.05);与对照组治疗后比较,治疗组血清IGF-1的升高更为明显,VEGF降低的更为明显(P<0.05)。结论加味补肝汤能有效改善DPN患者的临床症状,提高患者血清IGF-1水平、降低VEGF水平,可能为其治疗DPN机制之一。Objective To observe the therapeutic effect and neural protection mechanism of Jiawei Bugan Decoction（Modified decoction for reinforcing the liver）on the diabetic peripheral neuropathy（DPN）.Methods Eighty DPN patients were randomized into treatment group and control group with 40 in each.Forty healthy subjects were selected as normal control.The two intervention groups received basic treatment,the treatment group was additionally administered Jiawei Bugan Decoction,while the control group injected vitamin B1 and vitamin B12 intramuscularly.The treatment lasted 8 weeks in both groups.Before and after treatment,the levels of serum vascular endothelial growth factor（VEGF）and insulin-like growth factor 1（IGF-1）of the patients were detected,simultaneously the electromyographic nerve conduction velocity（NCV）was measured for observing the clinical efficacy.Results The total effective rate of the treatment group was 82.5%,significantly higher than that of the control group（57.5%,P0.05）.After treatment,NCV of both groups was significantly higher than before treatment（P0.05）,compared with the control group,the NCV increase in the treatment group was more significant（P0.05）.The level of IGF-1 was increased and the level of VEGF decreased in both groups than before treatment（P0.05）,compared with the control group after treatment,the IGF-1 increase and VEGF decrease in the treatment group were more significant（P 0.05）.Conclusion Jiawei Bugan Decoction functions to improve effectively the clinical symptoms and increase the IGF-1 level and decrease the VEGF level of the DPN patients.This may be one of its mechanisms in treating DPN.
