57 Cases Unstable Distal Radial Fracture Were Treated by Traditional Chinese Medicine and Western Medicine

摘  要:目的:总结中西医结合治疗桡骨远端不稳定性骨折的可行性和实效性。方法:2007年2月~2009年12月,采用掌侧入路切开复位,"T"型锁定钢板螺钉内固定,配合中草药熏洗治疗桡骨远端不稳定性骨折57例,其中"T"型锁定钢板螺钉内固定49例,锁定钢板螺钉加克氏针内固定2例,外固定架固定1例,有骨缺损自体髂骨植入的4例,人工骨植入的1例。结果:术后随访3~10个月,X线显示骨折全部愈合,优41例,良12例,可3例,差1例,优良率92.9%。结论:掌侧入路、"T"型锁定钢板螺钉内固定,手术创伤小,复位准确,固定可靠,便于操作;功能训练结合中草药熏洗,可减少粘连,有利于关节功能的恢复。Objective:To analyze the feasibility and effectiveness of unstable distal radial fracture treated by Traditional Chinese Medicine and Western Medicine.Methods:volar incision,open reduction,"T" type of locking plate and screw together with herbal fumigation were used in 57 cases,during february 2007-2009 december."T" Locking plate and screw were used in 49 cases.Locking plate screw and Kirschner wire fixation were used in 3 cases.Bone defects were filled with autologous iliac bone in 4 cases,1 case with bone implants.Results:The patients were followed up for 3-10 months(4 months averaged).Fractures were healed in X ray.41 cases were excellent,12 cases were good,3 cases were general and 1 case was poor;the fine rate was 92.9%.Conclusion:"T" type of locking plate and screw,with herbal fumigation treated the unstable distal radial fracture did not affect the bone and the sheath structure in distal radius.The plate were placed in favor,because the palm side of radius was flat.No damage to the intra-articular and palmar ligaments,it will be help for the functional recovery.Bone graft by the palm side,it will be help for the dorsal support.Chinese herbal Fumigation with functional training were conducive to the recovery of joint function reduce adhesion.
