血浆丙二醛和高敏C-反应蛋白与急性心肌梗死的相关性研究  被引量:14

摘  要:目的观察血浆丙二醛和高敏C-反应蛋白在冠心病人群中的变化,并深入探讨血浆丙二醛(MDA)、高敏C-反应蛋白(hs-CRP)与急性心肌梗死的相关性。方法分别选择稳定型心绞痛患者(稳定型心绞痛组)、不稳定心绞痛患者(不稳定心绞痛组)、急性心肌梗死患者(急性心肌梗死组)各40例,以及正常对照(正常对照组)20例,分别收集各组人群一般资料,并检测血浆常规生化指标、丙二醛、高敏C-反应蛋白等。结果与对照组相比,其余各组BMI、HDL-C、ApoA-1、ApoB水平显著升高(p<0.05);各组间血浆hs-CRP、MDA水平相比,差异有统计学意义(p<0.05);且从对照组,稳定型心绞痛组,不稳定性心绞痛组,到急性心肌梗死组呈递增趋势;logistic回归分析显示,血浆hs-CRP、MDA水平与急性心肌梗死正相关有统计学意义(p<0.05);血浆hs-CRP、MDA水平之间相关有统计学意义(p<0.05),进一步偏相关分析显示血浆hs-CRP与MDA水平之间具有直线相关性。结论这提示氧化应激、炎症反应可能参与急性心肌梗死的病理生理过程,而抗炎、抗氧化治疗可能对急性心肌梗死有益。Objective To investigate the levels of plasma malondialdehyde (MDA) and high sensitive C - reactive protein (hs- CRP)in coronary heart disease patients, and study the correlation of plasma MDA and hs- CRP in acute myocardial infarction patients in deep. Methods Selection 40 patients who diagnosed as stable angina peetoris, unstable angina pectoris, acute myocardial infarction, respectively, and taking 20 normal man as control. Then collection general characteristics and dectection some basic biochemical parameters, MDA, and hs - CRP of the study populations in each group. Results Compared with control group, the levels of BMI, HDL - C, ApoA - 1, ApoB were elevated in other groups (p 〈0. 05). And the levels of plasma hs - CRP, MDA have statistical difference among each group, and from the control group, stable angina pectoris group, unstable angina pectoris group, to acute myocardial infarction group showed increasing trends. The logistic regression analysis showed that the levels of plasma hs - CRP, MDA are positive correlation with acute myocardial infarction (p 〈 0.05 ), the correlation of the levels of hs -CRP and MDA are positive. Further partial correlation analysis showed that the levels of hs - CRP and MDA have the rectilinear correlation. Conclusion These results suggest that oxidative stress and inflammation may play important role in acute myocardial infarction, while anti -oxidant and anti -inflammatory may beneficial to acute myocardial infarction.
