摘　　要：目的:探讨醛固酮合酶基因单核苷酸多态性-344C/T与血管紧张素转换酶抑制剂(ACEI)降压疗效的相关性。方法:新发的轻中度原发性高血压患者121例,均来自北京市昌平地区的常住汉族人群,予以贝那普利口服治疗8w,利用TaqMan MGB等位基因分型试剂盒检测患者-344C/T基因多态性分布频率。结果:治疗后,TT基因型和CT基因型患者舒张压降低幅度分别为(13.50±4.91)mmHg(1mmHg=0.133 kPa)和(12.00±6.57)mmHg,高于CC基因型患者(8.64±6.74)mmHg,差异有统计学意义(P=0.045);经多元逐步回归分析,对可能影响ACEI类药物降压疗效的因素进行校正后,此多态性位点与治疗后舒张压的下降幅度有关(R2=0.052,P=0.023)。结论:醛固酮合酶基因-344C/T多态性可能与血管紧张素转换酶抑制剂的降压敏感性有关,T等位基因携带者服用贝那普利的降压效果更好。Objective：To investigate the association between the-344C/T polymorphism of the aldosterone synthase（CYP11B2） gene and the blood pressure response to angiotensin-converting enzyme inhibitors in patients with essential hypertension.Methods： 121 patients with mild to moderate essential hypertension were included in this study.Benazepril was administered to the patients for 8 weeks.Genotyping was performed to identify the-344C/T polymorphisms of the CYP11B2 gene.Results： After 8 weeks of treatment,the reductions in diastolic blood pressure were significantly greater in patients carrying the TT or CT genotype compared with those carrying the CC genotype [（13.50±4.91）mmHg（1 mmHg=0.133 kPa） or（12.00±6.57）mmHg versus（8.64±6.74）mmHg,respectively;P=0.045）.Stepwise multiple regression analysis showed that there was significant association between-344C/T polymorphism and the reductions in diastolic blood pressure（R2=0.052,P=0.023,）.Conclusion： The-344C/T polymorphism of the CYP11B2 gene may be a determinant of the blood pressure response to angiotensin-converting enzyme inhibitors in patients with essential hypertension.
