双吻合器技术在低位直肠癌保肛术中的应用价值

摘　　要：目的探讨双吻合器技术在低位直肠癌手术中的应用及疗效。方法对2000年1月～2007年1月间低位直肠癌患者中采用双吻合器行保肛术137例(观察组)与同期98例手工缝合行Dixon手术(对照组)的临床资料进行比较。结果应用双吻合器组保肛率明显高于Dixon手术组(p<0.05),手术时间、术后吻合口漏、吻合口狭窄及出血发生率则明显低于Dixon手术组(p<0.05),而两组手术过程中输血量比较,差异无统计学意义(p>0.05),术后3个月时应用双吻合器组患者的控便能力正常比例明显高于Dixon手术组,差异具有统计学意义(p<0.05),随访期间,两组局部复发率及1、3、5年生存率比较,差异无统计学意义(p>0.05)。结论双吻合器技术应用可提高低位直肠癌的保肛率及生存质量,是一种安全可靠的方法。Objective To study and evaluate the role of double stapling procedure for the anus preserving re- section of low rectal carcinoma. Methods The clinical materials of dixon group （98 cases） and double stapler group （ 137 cases）for the resection of low rectal carcinoma were comparatively analyzed. Results The anus reserving rate of double stapler group is obviously higher than that of dixon group（p 〈 0. 05 ）. The operation time, the ration of anasto- motic leakage and anastomotic stenosis of double stapler group was lower than dixon group （p 〈 0. 05 ）. There was no statistical difference between these two group of transfusion volume（p 〉 0.05 ）. The ratio of normal ability of control-ling stool was higher than dixon group（p 〈 0. 05 ）. There was no statistical difference between these two group of the 1.3.5 year survival rate of post - operation （p 〉 0. 05 ）. Conclusion The double stapling technique can improve the quality of life of patients and the rate of reserving anus, is a safe alternation for anus - saving operation for patients with low rectal cancer.
