摘　　要：目的:探讨齐拉西酮合并小剂量氯氮平治疗难治性精神分裂症的临床疗效观察及护理方法。方法:将120例难治性精神分裂症患者随机分为两组,每组各60例,治疗组给予齐拉西酮合并小剂量氯氮平治疗,对照组给予氯氮平治疗,疗程均为12周。采用阳性与阴性症状评定量表(PANSS)及症状量表(TESS)于治疗前、治疗后1、2、4、8、12周末各评定1次。结果:从治疗的第2周末起,两组PANSS的T、P、N、G因子分值分别和治疗前比较有显著性差异(P<0.05),治疗4周后,差异均有非常显著性(P<0.01);在治疗第4周末,两组间PANSS的T、G因子分的差异有显著性(P<0.05);在治疗8周末后,两组间PANSS的T、P、N、G因子分的差异有显著性(P<0.01);治疗12周后,两组总有效率差异有显著性(P=0.027),且治疗组的不良反应较对照组轻。结论:齐拉西酮合并小剂量氯氮平治疗难治性精神分裂症的疗效显著,安全性高,减轻护理工作量,提高了护理质量,值得在临床上推广使用。Objective To investigate the combined low-dose ziprasidone clozapine treatment of refractory schizophrenia clinical observation and care methods.Methods 120 patients with refractory schizophrenia were randomly divided into two groups of 60 patients,combined treatment group received low-dose clozapine ziprasidone treatment,control group was given clozapine treatment,a course of 12 weeks.The use of positive and negative symptoms rating scale（PANSS） and the symptom scale（TESS） before treatment,after treatment 1,2,4,8,12 week 1 of each assessment.The results from the 2nd week of treatment,the two PANSS of T,P,N,G,and factor scores before treatment were significant differences（P0.05）,4 weeks after treatment,the differences were very significant（P0.01）;the first 4 weeks of treatment,PANSS between the two groups of T,G factor scores were significantly different（P0.05）;8 weeks of treatment, the PANSS between the two groups of T,P,N,G factor score difference was significant（P0.01）;after 12 weeks of treatment, total effective rate was significant difference（P = 0.027）,and the treatment group than in the control group with mild adverse reactions. Conclusion with small doses of ziprasidone clozapine treatment of refractory schizophrenia is significant,high security,reduce nursing workload and improve the quality of care,it is used in clinical practice.
