老年患者腹腔镜胆囊切除术后的护理干预

摘　　要：目的探讨老年患者腹腔镜胆囊切除术(LC)后的护理措施。方法对淮北市人民医院2008年12月-2010年12月收治的40例65岁以上的老年患者均行腹腔镜胆囊切除,同时术后给予积极护理干预的临床资料进行回顾分析。结果胆囊结石25例,胆囊息肉10例,结石合并息肉5例;合并高血压4例,合并心脏病7例,合并糖尿病9例,合并慢性支气管炎6例,合并前列腺增生8例。40例患者手术顺利,术后胆瘘1例,术后腹腔出血1例,术后呼吸道感染4例,术后排尿困难5例,经积极治疗及护理,39例3～7 d出院,1例16 d后出院。结论术后合理的呼吸道护理及饮食指导,积极观察生命体征变化,及时有效的并发症护理及心理护理对患者术后恢复起重要作用。Objective To explore the nursing measure for elderly patients after laparoscopic cholecystectomy. Methods The clinical data of 40 senile patients with laparoscopic cholecystectomy from Dec. 2008 to Dec. 2010 were analyzed retrospectively. The active nursing intervention was performed after the operations. Results In all patients, there were 25 cases with cholecystolithiasis,10 with gallbladder polyps,5 cases with cholecystolithiasis and gallbladder polyps simuhaneously,4 with hypertension,7 with cardiac diseases ,9 with diabetes ,6 with chronic bronchitis and 8 with prostatic hyperlasia. All the operation was successfully performed. After the operation, 1 case was biliary fistula, 1 with intraperitoneal hemorrhage ,4 cases with respiratory infection and 5 cases with dysuresia. 39 patients were discharged in 3 to 7 days, and 1 in 16 days. Conclusion The rational nursing management for respiratory tract and diet instruction, active observation of vital signs, punctual and efficient nursing of complications are critical to the elderly patients after laparoscopic cholecystectomy operation.
