600例急诊手外伤患者围术期预防性应用抗菌药物转换疗法调研分析

摘　　要：目的:了解北京积水潭医院急诊手外伤患者围术期预防性应用抗菌药物转换疗法的情况,探讨其合理性。方法:对我院2010年1—6月急诊手外伤600例围术期患者的病历进行回顾性调查分析。结果:600例急诊手外伤手术患者中Ⅰ类切口597例,Ⅱ类切口1例,Ⅲ类切口2例;有367例(61.17%)使用了转换疗法,未使用的患者有233例(38.83%);在抗菌药物的选择、疗程、转换时间、转换疗程等方面还存在不合理现象。结论:抗菌药物的转换疗法可用于急诊手外伤术后预防感染,是有效的治疗方案,但应提高合理应用抗菌药物的水平,进一步完善围术期预防性应用抗菌药物的转换疗法,使其能更好地应用于临床。OBJECTIVE： To evaluate the rationality of the prophylactic antibiotic switch therapy used perioperatively for patients with emergency hand trauma in Beijing Jishuitan Hospital.METHODS： The medical records of 600 patients undergoing emergency hand trauma surgery patients in our hospital between Jan.and Jun.2010 were reviewed retrospectively.RESULTS： Of the total 600 cases,597 had type Ⅰ incision,1 type Ⅱ and 2 type Ⅲ incisions.367（61.17%） received switch therapy and 233（38.83%） did not receive switch therapy.The irrationality in antibiotics use was noted in the selection of antibiotics,course of treatment,switching time,switching of treatment course.CONCLUSION： Antibiotic switch therapy is effective for infection prevention in patients with emergency hand trauma.However,rational use of antibiotics and perioperative use of prophylactic antibiotic switch therapy remain to be improved for clinical benefit.
