摘　　要：目的:了解苏州大学附属常熟医院"特殊使用"抗菌药物的应用趋势,为减少抗菌药物引发不良反应、避免细菌过快产生耐药性提供参考。方法:采用药物利用研究中的限定日剂量(DDD)、销售金额、用药频度(DDDs)等指标,对我院2007—2010年"特殊使用"抗菌药物的应用情况进行分析。结果:第4代头孢菌素的销售金额、DDDs所占比例最高;不同类别"特殊使用"抗菌药物品种,其销售金额、DDDs排序及所占比例等指标变化各不相同;"特殊使用"抗菌药物的销售金额排序与DDDs排序比介于0.33～9.00之间;94.74%的"特殊使用"抗菌药物在我院均有应用;部分"特殊使用"抗菌药物对主要目标菌的耐药率在90%以上。结论:针对一定程度的抗菌药物滥用问题,应在我院采取严格的限制措施。OBJECTIVE： To understand the utilization trend of antibiotics for special use in Changshu Hospital Affiliated to Soochow University in order to provide reference for reducing incidence of antibiotics-induced ADR and avoiding rapid occurrence of bacterial resistance.METHODS： The utilization of antibiotics for special use in our hospital from 2007 to 2010 was analyzed with defined daily dose（DDD）,consumption sum and DDDs as indexes.RESULTS： The forth generation cephalosporins showed the highest proportion in both consumption sum and DDDs.The changes of antibiotics for special use in consumption sum,DDDs order and proportion varied from each other.The ratio of consumption sum order to DDDs order of antibiotics for special use was in a range of 0.33-9.00.94.74% of antibiotics for special use have been used in our hospital.The resistance rates of major targeted bacteria to some antimicrobials for special use were above 90%.CONCLUSION： Strict measure should be taken in our hospital to tighten control on the abuse of antibiotics.
