阿德福韦和恩替卡韦治疗慢性乙型肝炎144周后的疗效比较

摘　　要：目的比较分析阿德福韦(ADV)、恩替卡韦(ETV)治疗慢性乙型肝炎(CHB)144周的临床疗效。方法入组103例CHB随机分为两组,其中ADV组47例,ETV组56例,分别给予ADV和ETV治疗。治疗前后分别检测两组患者HBV DNA、HBV血清学标志物及生化指标。结果治疗后,ETV组和ADV组各检测时点HBV DNA不可测率差异均有统计学意义(P<0.05);两组高、低病毒载量患者各检测时点HBV DNA不可测率差异亦有统计学意义(P<0.05);两组患者各时点的ALT复常率及HBeAg、HbeAb血清转化率差异无统计学意义(P<0.05)。两药的耐药性和不良反应差异无统计学意义(P>0.05)。结论 ETV较ADV抑制病毒速度快;两者144周内的耐药性相似,且对HBeAg和HBeAb血清转换率均呈逐年上升趋势。Objective To compare the clinical efficacy of adefovir（ADV） and entecavir（ETV） in the treatment of chronic hepatitis B（CHB） after 144 weeks.Methods One hundred and three patients with CHB were randomly divided into two group.47 patients in the ADV group and 56 patients in the ETV group were respectively treated with ADV and ETV.Before and after treatment,HBV DNA,HBV serology markers and biochemical markers were detected.Results After the treatment,there have significant differences in the undetectable rates of all the time points between the two group（P0.05）;There have significant differences in the undetectable rates of all the time points between the patients with high viral load of the two group（P0.05）;There have significant differences in the undetectable rates of all the time points between the patients with low viral load of the two group（P0.05） .There have no significant differences in the ALT normalization rate,and the seroconversion rates of HBeAg and HBeAb of all the time points between the patients with low viral load of the two group（P0.05） .The resistance and toxicity of ADV and ETV were not significantly different（P0.05） .Conclusions The viral suppression speed of ETV is faster than that of ADV;The resistances of the two drugs are similar within 144 weeks;The seroconversion rate of HBeAg and HBeAb showed an increasing trend.
