摘 要:目的探讨麝香保心丸(HMP)对心梗后心衰家兔心肌细胞外间质重构的影响及其可能机制。方法结扎家兔冠状动脉左前降支建立心梗心衰模型。术后2周随机分为心衰4周(CHF 4 W)及8周组(CHF 8 W)、麝香保心丸4周(HMP 4 W)及8周组(HMP 8 W)、依那普利4周(ENA 4 W)及8周组(ENA 8 W),同时设假手术组(Sham)为对照。测定血流动力学指标、左室质量指数(LVMI)、心肌胶原容积分数(CVF),RT-PCR检测心肌基质金属蛋白酶-9(MMP-9)及其内源性组织抑制因子-1(TIMP-1)mRNA表达。结果与CHF组比较,经HMP和ENA治疗后CO、CI、全心射血分数(GEF)升高(P<0.05),全心舒张末期容积指数(GEDI)、LVMI及CVF降低(P<0.05),以HMP8W和ENA8W组改善为著。CHF组MMP-9 mRNA表达水平升高,MMP-9/TIMP-1 mRNA比值增大。经HMP及ENA治疗后,心肌MMP-9及TIMP-1 mRNA表达水平下降,两者比值保持动态平衡。结论 HMP与ENA一样,具有遏制和减轻心梗后心衰心肌细胞外间质重构作用,其机制可能与降低心肌MMP-9和TIMP-1 mRNA表达水平,保持两者比值动态平衡相关。AIM To investigate the effects and mechanisms of Heart-protecting Musk Pill(HMP) on myocardial interstitium reconstruction in rabbits with heart failure(CHF) after myocardial infraction(MI).METHODS We established the CHF models after MI in New Zealand rabbits by ligation of the left anterior descending coronary artery.Rabbits with CHF were randomly divided into CHF 4 W group,CHF 8 W group,Enalapril(ENA) 4 W group,ENA 8 W group,HMP 4 W group and HMP 8 W group.Meanwhile sham operation(Sham) group was set up to serve as controls.The effects on such indexes as hemodynamic state,left ventricular mass index(LVMI),myocardial collagen volume fraction(CVF) and the mRNA expression levels of MMP-9 and its endogenous tissue inhibitor-1(TIMP-1) mRNA were determined after treatment for 4 or 8 weeks.RESULTS The CO,CI and global ejection fraction(GEF) in HMP and ENA groups were higher than these in CHF groups(P〈0.05).While global end diastolic volume index(GEDI),LVMI and CVF decreased in HMP and ENA groups(P〈0.05),with more significant improvement in HMP 8 W and ENA 8 W groups.The mRNA expression levels of MMP-9 increased and MMP-9/TIMP-1 ratio increased in CHF groups.The mRNA expression levels of MMP-9 and TIMP-1 were decreased in HMP groups and ENA groups than those in CHF groups,with the ratio between the two maintaining a dynamic balance ratio.CONCLUSION HMP and ENA have the same effect on alleviating myocardial extracellular matrix remodeling in rabbits with heart failure.The mechanism may be related to the reduction of the mRNA expression levels of MMP-9 and TIMP-1,and maintenance of the ratio between the two at a dynamic balance.
