Philos钢板治疗肱骨近端骨折的临床应用

摘　　要：目的探讨应用AO肱骨近端内固定锁定系统(Philos)治疗肱骨近端骨折的临床疗效。方法我院2007年1月至2010年2月间使用Philos钢板治疗肱骨近端骨折25例,根据Neer分型:二部分骨折10例,三部分骨折12例,四部分骨折3例,其中3例骨折累及肱骨干上段。术后悬吊固定3周左右,术后3d开始适量肩关节被动运动,2周后适量主动活动,3周后加强主动功能锻炼。结果全病例部获得随访,随访时间10～16个月,平均13.1个月,骨折均愈合,平均临床愈合时间12.6周(10～16周)。按Constant评分标准评定,优11例,良8例,优良率为76%。结论 Philos钢板是治疗肱骨近端骨折特别是骨质疏松性骨折的理想选择。Objective To discuss the clinical effect of open reduction and internal fixation with proximal humeral internal locking system（Philos）on proximal humeral fractures.Methods 25 cases with proximal humeral fractures treated with Philos from January 2007 to February 2010 were studied retrospectively.According to Neer classification,there were 10 cases with two-part fractures,12 cases with three-part fractures and 3 cases with four-part fractures including of 3 cases with fracture line extending to the humeral shaft.Passive shoulder exercise could be performed three days after operation.After two weeks,active shoulder exercise was done,which was gradually intensified three weeks after operation.Results The follow up period was 10-16 months（mean 13.1 months） and all fractures were healed with average union period of 12.6 weeks（10~16 weeks）.According to the constant shoulder score,11 cases were excellent,8 cases were good,with all excellence rate of 76%.Conclusion Philos plate is a good selection for proximal humeral fractures and better for the fracture with osteoporosis.
