摘 要:目的血浆网膜素(Omentin)是新近发现的一种脂肪细胞因子,本研究旨在探讨重组促胰岛素分泌素(商品名Byetta)对2型糖尿病(T2DM)患者血浆Omentin水平的影响。方法采用病例对照及自身前后对照研究设计,28例初诊T2DM患者和28例性别、年龄相匹配的正常人(NGT)作为研究对象。给予T2DM患者Byetta治疗12周,采用酶联免疫法测定T2DM患者及正常人血浆Omentin水平,并分析Byetta治疗前后血浆Omentin水平的变化及其与BMI、WHR、胰岛素抵抗指数(HOMA-IR)等代谢指标变化的关系。结果 T2DM组空腹血浆Omentin水平明显低于正常对照组(NGT组)([18.5±4.3)vs(23.2±5.5)μg/L,P<0.01)]。Byetta治疗12周后,空腹血浆Omentin水平较治疗前显著升高([18.5±4.3)vs(22.7±4.6)μg/L,P<0.01]。血浆Omentin的升高的程度与HOMA-IR降低程度呈显著正相关(r=0.54,P<0.05)。相关分析提示T2DM患者空腹血浆Omentin水平与FINS和HOMA-IR呈明显负相关。多元逐步回归分析FINS、HOMA-IR是影响T2DM患者血清Omentin水平的相关因素。结论 Byetta能有效改善T2DM患者糖代谢和胰岛素敏感性,同时升高血浆Omentin水平。Objective Omentin has recently been charactered as a newly lipid metabolism regulator.This study aimed to investigate the effects of Byetta on plasma Omentin levels in patients with type 2 diabetes mellitus(T2DM).Methods This is a case-control study.Twenty-eight patients with T2DM(T2DM) and 28 sex-and age-matched normal glycaemic controls(NGT) were enrolled in the study.The T2DM group was treated with Byetta for 12 weeks.Plasma Omentin levels were measured by ELISA in patients with T2DM and NGT.Omentin levels between pre-and post-treatment with Byeeta were compared and the relationship between plasma Omentin levels and BMI,WHR,HOMA-IR,and other metabolic parameters were also analyzed.Results Omentin levels were lower in T2DM group than that in the control(18.5±4.3 vs 23.2±5.5μg/L,P0.01).Compared with pre-treatment,fasting plasma Omentin levels were increased significantly in T2DM group after treatment with Byetta(18.5±4.3 vs 22.7±4.6μg/L,P0.01)and the change of Omentin were correlated with the change of HOMA-IR(r=0.54,P0.05).In T2DM patients,Plasma Omentin levels was negatively with FINS and HOMA-IR.Multiple regression analysis showed that FINS and HOMA-IR were associated with plasma Omentin levels.Conclusion Byetta can remarkably improve glucose metabolism and insulin sensitivity,and also increase plasma omentin levels.
