藏族地区基层医院开展腹腔镜手术的应用体会

摘　　要：目的探讨藏区基层医院开展腹腔镜手术(laparoscopic operation,LO)的经验和体会,并评估其临床应用价值。方法回顾分析2008年3月～2011年4月初期开展的279例腹腔镜手术的临床资料。结果完成腹腔镜手术265例(95.0%),中转开腹10例(3.6%),二次手术4例(1.4%);并发胆漏4例(1.4%),发生肝总管以上部位的高位胆管损伤2例(0.7%),均经过积极处理后没有严重后遗症发生。结论藏区基层医院开展LO,必须高度重视,严格人员培训,重视胆囊三角的解剖,严防胆管损伤,适时中转开腹。循序渐进地开展腹腔镜手术是安全、可行的,值得临床推广应用。Objective To explore the experience and realization of laparoscopic operation in the primary hospital of the Tibetan area,assess the value to apply this technique.Methods The clinical data of 279 patients treated by laparoscopic operation the beginning from Mar.2008 to Apr.2009 were retrospectively analyzed.Results LO was successfully accomplished 265 cases（95.0%） and the conversion to opened operation was required in 10 cases（3.6%）.4 cases（1.4%） were treated by the second operation;4 cases（1.4%） suffered from bile leakage,2 cases were complicated with bile duct injury above the hepatic duct（0.7%）,but all patients were treated well and recovered without serious sequelae.Conclusion LO can be carry in the primary hospital of the Tibetan area,but they must strengthen the training of operators,take care of the anatomy of calot＇s triangle,avoid the bile duct injury and convert to open surgery appropriately.It is safe and feasible to develop laparoscopic operation step by step and a great of value to apply this technique.
