培哚普利对压力超负荷性左室肥厚的防治作用

摘　　要：目的　研究培哚普利防治压力超负荷性左室肥厚的作用及其机制。方法　术后 6周后测定平均动脉压 (MAP)、左室重量 /体重比值 (LVW /BW) ,图像法测算出心肌间质胶原蛋白总体积 (V(c) t)和每一心肌细胞核所在的心肌细胞的平均体积 (V(m) n) ,放免法测定心肌血管紧张素Ⅱ浓度 (MAⅡ )。结果　和假手术组相比 ,腹主动脉缩窄组MAP、V(c) t、LVW /BW、V(m) n和MAⅡ均显著增加 ;低剂量组MAP显著增高 ,余指标无显著差异 ;高剂量组各指标无显著性差异。LVM/BW和MAⅡ高度相关 (P <0 .0 1 )。结论　高、低剂量培哚普利均可防止心肌肥厚和间质胶原蛋白的沉积 ,其机制可能是抑制了MAⅡ的产生。Objective The effect of perindopril on preventing pressure overload left ventricular hypertrophy and its mechanism was studied. Methods In the sixth week after operation ,mean arterious pressure (MAP) and ratio of left ventricular weight to body weight (LVW/BW)were examined. Total intestitial collagen volume (V(c)t)and average myocyte volume per nucleus (V(m)n)were measured and calculated by morphometry. Concentration of myocardial angiotensin Ⅱ(MAⅡ)were determined by radioimmunoassay. Results Compared with sham operation group, MAP ,V(c)t, LVW/BW, V(m)n and MAⅡ in contricted abdominal aorta group were significantly increased; MAP in low dose group was significantly higher than that in sham operation group. The other variables in low dose group were similar to those in sham operation group. There were not statistical differences between sham operation group and high dose group. There was only a significant correlation of MAⅡ with LVW/BW(P<0.01). Conclusions Both high dose and low dose perindopril prevent myocyte from hypertrophy and accumulation of collagen in intestium. The possible mechanism is that perinsopril inhibits the production of MAⅡ.
