单孔内镜经腋窝行单侧甲状腺切除手术的临床研究

摘　　要：目的探讨单孔腹腔镜经腋窝行单侧甲状腺切除手术的临床效果及可行性。方法收集2009年4月～2010年10月该院行单侧甲状腺良性疾病手术患者64例,其中单孔腹腔镜经腋窝行单侧甲状腺切除手术24例(研究组),传统开放甲状腺手术患者40例(对照组)。分别比较两组患者的手术时间、术中失血量、术后止痛药使用率、术后引流量、术后住院时间、术后并发症发生情况、复发率和美容效果满意度。结果研究组和对照组患者的手术时间、术中失血量、术后引流量及术后并发症发生情况比较,差异无显著性(P>0.05),而研究组术后止痛药使用率明显低于对照组,术后住院时间明显短于对照组(P<0.05)。研究组和对照组患者的术后复发率比较,差异无显著性(P>0.05);研究组患者对术后切口美容满意度明显高于对照组患者(P<0.05)。结论单孔腹腔镜经腋窝行单侧甲状腺切除手术是一种安全可行的手术方式,具有创伤小、术后疼痛轻、术后恢复快、美容效果极佳的潜在优势。[Objective] To explore the feasibility and safety of single port laparoscopic thyroidectomy in patients with nodular goiter.[Methods] Twenty four patients underwent single port laparoscopic thyroidectomy and 40 patients underwent conventional open thyroidectomy were collected in this study and was named study group and control group respectively.The operating time,postoperative drainage volume,consumption of analgesics,intraoperative blood loss,postoperative hospital stay,postoperative complications,recurrence rate and cosmetic results were compared between the two groups.[Results] There were no significant difference in operating time,postoperative drainage volume and intraoperative blood loss between the two groups（P 0.05）.The consumption of analgesics in study group was lower significantly than control group（P 0.05）.The postoperative hospital stay in study group was shorter significantly than control group（P 0.05）.There was no significant difference in recurrence rate between the two groups（P 0.05）.The satisfactory cosmetic results in study group was higher significantly than control group（P 0.05）.[Conclusions] single port laparoscopic thyroidectomy in patients with nodular goiter is safe and effective,and it has some advantages such as small incision,faster recovery,minimally invasive,and better cosmetology.
