摘 要:目的观察静脉注射外源性硫化氢(H2S)对异丙肾上腺素(ISO)诱导的大鼠急性心肌缺血的保护作用。方法健康雄性SD大鼠30只,体质量200—250g,随机分为三组(n=10):正常对照组、模型组和NaHS组。模型组和NaHS组采用皮下多点注射ISO方法制作大鼠急性心肌缺血模型。对照组皮下注射等容积生理盐水(0.1mL/100g)。NaHS组静脉注射NaHS(50mmol/L,0.1mL/100g),模型组和正常对照组静脉注射等容积生理盐水(0.1mL/100g)。给药后,记录二三组大鼠70min内心电图。实验结束后,各组大鼠取血测定血清丙氨酸转氨酶(ALT)、天冬氨酸转氨酸(AST)、磷酸肌酸激酶(CK)、磷酸肌酸激酶同工酶(CK—MB),随后处死大鼠,分离大鼠心脏,切片,做病理学检查。结果模型组给药后10min心电图ST段显著压低[模型组(-0.21±0.20)mV比对照组(0.04±0.04)mV,P〈0.05]。NaHS组心电图ST段在给药后30min开始抬高[模型组(-0.18±0.16)mV比NaHS组(-0.09±0.06)mV,P〉0.05],50rain基本恢复正常,与模型组比较差异有统计学意义[模型组(-0.19±0.17)mV比NaHS组(-0.01±0.06)mV,P〈0.05]。NaHS组血清ALT、AST、CK、CK—MB水平显著低于模型组(P〈0.05)。模型组心肌病理学检查可见显著心肌间质水肿和心肌细胞颗粒变性及嗜酸性变,NaHS组心肌间质未见显著水肿,心肌细胞颗粒变性和嗜酸性变程度显著减轻。结论外源性H2S对异丙肾上腺素诱导的大鼠急性心肌缺血有显著的保护作用。Objective To observe the protective effect of hydrogen sulfide on the acute myocardial ischemia induced by isoprenaline (ISO) in the rats. Methods Thirty male SD rats, weighing 200 -250 g, were randomized into 3 groups (n =10) : control group, model group and NariS group. Acute myocardial ischemia was induced by multi - point subcutaneously injection of ISO in rats of model and NariS group, Saline (0. 1 mL/100 g) or NariS (50 retool/L, 0. 1 mL/100 g) was administrated comparatively in control group or NariS group. Electrocardiogram (ECG) was recorded about 70 rains. Serum alanine aminotransferase ( ALT), aspartate aminotransferase ( AST), creatine kinase(CK) and creatine kinase isoenzyme (CK- MB ) were measured. The pathological changes of heart tissue were determined. Results After recording 10 mins, ST segment of rats decreased obviously in model group than that in control group (P 〈0.05). ST segment began to rise in NariS group after 30 rain and recovered within 50 min compared with model group ( P 〈 0.05 ). The serum levels ALT, AST, CK and CK - MB were lower in Naris group than those in model group. Pathology examination indicated obvious myocardial edema, myocardial cells degeneration and acidophilic degeneration in model group.NariS group had no obvious myocardial edema, myocardial cells degeneration and acidophilic degeneration. Conclusion Hydrogen sulfide has an obviously protective effects on the rats of acute myocardial ischemia induced by ISO.
