摘 要:目的:探讨非ST段抬高急性冠脉综合征(ACS)患者高敏C反应蛋白(hs-CRP)与抵抗素、脂联素(Adp)的关系及临床意义。方法:选取非ST段抬高ACS的患者106例,分为不稳定性心绞痛(UA)组69例及急性非ST段抬高心肌梗死(NSTEMI)组37例;另入选稳定型心绞痛(SAP)组35例及健康对照组35例。所有入选患者行总胆固醇(TC)、低密度胆固醇(LDL)、高密度胆固醇(HDL)、甘油三酯(TG)及hs-CRP检测,并测定Adp及抵抗素水平。结果:与SAP组比较,UA组及NSTEMI组抵抗素水平增高,Adp减低(P<0.05)。NSTEMI组hs-CRP水平较健康对照组及SAP组明显增高(P<0.05)。非ST段抬高ACS患者中,高危险组hs-CRP及抵抗素水平增高,Adp减低(P<0.05);相关性分析显示,Adp与抵抗素呈负相关(r=-0.829,P<0.001),抵抗素与HDL呈负相关(r=-0.312,P=0.047),Adp与HDL呈正相关(r=0.304,P=0.053)。结论:高危非ST段抬高ACS患者hs-CRP、抵抗素水平明显高于低危险组,而Adp水平显著低于中低危险组;hs-CRP、抵抗素、Adp水平与冠状动脉斑块稳定性有关。Objective To investigate the association between high-sensitivity C-reactive protein (hs-CRP) and adiponectin, resistin in patients with non-ST elevated acute coronary syndrome (ACS). Methods One hundred and six patients with non-ST elevated ACS were divided into two groups: unstable angina group (UA group, n = 69) and non-ST elevated myocardial infarction group (NSTEMI group, n = 37). Another 35 patients with stable angina pectoris (SAP group) and 35 healthy controls (control group) were also included in the study. Levels of total cholesterol (TC), low density cholesterol (LDL), high density cholesterol (HDL), triglyeeride (TG), (hs-CRP), adiponeetin, and resistin were measured. Results As compared to that in SAP control group, the level of resistin was increased, while adiponeetin was decreased in UA group and NSTEMI group (P 〈 0.05). Level of hs-CRP in NSTEMI group was significantly higher than those in control group and SAP group (P 〈 0.05). In NSTEMI group, hs-CRP and resistin were increased, whilst adiponectin was decreased in patients in high-risk subgroup as compared to those in low-risk subgroup (P 〈 0.05). Correlation analysis showed significant correlations between adiponectin and resistin (r = -0.829, P 〈 0.001 ), between resistin and HLD (r = -0.312, P = 0.047), and between adiponectin and HDL (r = 0.304, P = 0.053). Conclusion Levels of hs-CRP, adiponeetin, and resistin are associated with the stability of the plaque in coronary artery.
