早期肠外营养支持对重型颅脑损伤患者术后蛋白质代谢的影响

摘　　要：目的:观察早期肠外营养支持对重型颅脑损伤术后患者蛋白质代谢的影响。方法:将40例重型颅脑损伤术后患者随机分为两组,观察组20例,术后即开始给予营养配制液行肠外营养支持;对照组20例,术后给予5%葡萄糖注射液、复方氨基酸、30%脂肪乳分别静脉滴注。治疗后对其相关指标进行检测和评估。结果:观察组治疗第7天时前白蛋白指标较对照组明显改善,第14天时前白蛋白、转铁蛋白、白蛋白指标均较对照组明显改善,负氮平衡转为正氮平衡,而对照组仍处于负氮平衡;观察组治疗第14天时GCS评分较对照组明显升高。结论:早期合理的肠外营养支持对改善重型颅脑损伤患者术后营养状况及神经功能恢复有积极的作用。Objective：To observe the early treatment effect of parenteral nutrition on Metabolism of protein after the operation for patients with severe head injury.Methods：40 cases of severe head-injured were randomLy divided into two groups each 20： the observation group,in which parenteral nutrition configuration liquid was given after the operation,or the control group,in which 5% glucose,amino acids,30% fat milk were injected through vein.The serum level of prealbumin,transferrin and albumin were individually examined and evaluated after treatment.Results：The serum level of prealbumin in the observation group was higher than that before treatment,and increased as compared with the controls on the seventh day of treatment,and serum level of prealbumin,transferrin,and albumin were significantly increased compared with control group on the 14th day of treatment.Moreover,the blood nitrogen balance changes from negative to positive status,and the GCS score was higher in the observation group but no change in the control group on the 14th day of treatment.Conclusions：The parenteral nutrition has an active effect on Post-operative severe head injury at early administration.
