昼夜血压变化和糖尿病患者自主神经病变相关性的探讨

摘　　要：目的监测血液昼夜变化有助于早期发现和针对性治疗糖尿病自主神经病变的患者。方法选取38例血压正常的2型糖尿病患者38例，监测24小时动态血压，并且与38例正常对照组相比。同时将38例糖尿病患者按有无自主神经病变再分为无自主神经病变组18例与有自主神经病变组20例，观察两组心血管自主神经功能测定的结果以及夜间血压下降的情况。结果与对照组相比，糖尿病组的患者的平均24小时、白昼、夜间收缩压明显增高（P〈0．01），平均24小时、夜间舒张压明显增高（P〈0．05，P〈0．01），而平均白昼舒张压无明显变化（P〉0．05）；夜间收缩压及舒张压的下降百分率均明显降低（P〈0．01，P〈0．05）；而有自主神经病变组与无自主神经病变组相比，夜间收缩压下降百分率明显降低（P〈0．01），而夜间舒张压下降百分率无明显变化（P〉0．05）。结论糖尿病患者血压昼夜节律改变与是否合并自主神经病变有密切的关系。Objective The purpose to early detection and treatment of patients with diabetes independent neuropa- thy.Methods 38 cases of normal blood pressure patients with type 2 diabetes patients,and 24- hour ambulatory blood pressure monitoring,and compared with the normal control group of 38 patients. While 38 patients with diabetes patients without neuropathy by independent independent neuropathy is divided into 18 cases with independent group, the group of 20 cases of neuropathy obser- vation results of two groups CANF nighttime blood pressure drop and the situation. Results compared with the control group, dia- betes patients, the average 24 hours day,night significantly higher systolic blood pressure（ P 〈 0.01 ）, average 24 hours, nocturnal diastolic pressure increased significantly（ P 〈 0.05, P 〈 0.01）, and the average day diastolic pressure without obvious changes（ P 〉 0.05）.Systolic blood pressure and diastolic blood pressure drop percentage of night are decreased significantly（ P 〈 0.01, P 〈 0.05） .But have independent neuropathy with no independent neuropathy in night fell signi^cantly lower systolic percentage（ P 〈 0.01） ,and nocturnal diastdic blood pressure no apparent change of drop percentage（ P 〉 0.05）. Conclusion diabetic patients with blood pressure circadian rhythms are merged independent neuropathy is this close relatiomhip.
