动态监测血清CA-125、β-HCG、孕酮在预测早期先兆流产结局中的临床价值

摘　　要：目的探讨动态监测血清CA-125、β-HCG、孕酮在预测早期先兆流产结局中的临床价值。方法对96例早期先兆流产孕妇治疗前、治疗后1、2、4周的血清CA-125、β-HCG、孕酮进行测定,根据妊娠结局分为继续妊娠组及难免流产组。同时选择30例正常妊娠孕妇作为对照组。结果难免流产组血清CA-125检测值各时段均明显高于继续妊娠组及对照组,而β-HCG及孕酮各时段均明显低于继续妊娠组及对照组,统计有显著性差异。继续妊娠组CA-125治疗前高于对照组,而β-HCG及孕酮治疗前低于对照组,统计有显著性差异。治疗后2周,继续妊娠组与对照组各项数值趋于相同。结论单次CA125检测可快速预测早期先兆流产结局,而联合动态监测β-HCG、孕酮可提高其预测准确性。Objective To explore the value of monitoring serum CA-125、β-HCG and progesterone continuously in predicting the outcome of threatened abortion.Methods The serum CA-125、β-HCG and progesterone levels of 96 patients with threatened abortion were measured before treatment and 1,2,4 weeks after treatment.According to their outcomes,the patients were divided into inevitable abortion group and evitable abortion group.At the same time,30 normal pregnant women were selected as contrastive group.Results The serum CA-125 level of the inevitable abortion group was significantly higher than that of evitable abortion group or contrastive group,while theβ-HCG and progesterone level were significantly lower than those the other two groups at all times（P〈0.01）.The serum CA-125 level of the evitable abortion group was higher than that of contrastive group before treatment,while theβ-HCG and progesterone level was lower than that of contrastive group before treatment（P〈0.05）.2 weeks after treatment,all serum levels of the evitable abortion group and the contrastive group were no significant difference.Conclusions By a single serum CA125 level,we can predict the outcome of threatened abortion fast and early.By combining dynamic observation of serum β-HCG and progesterone,we can improve the accuracy.
