老年患者静脉穿刺困难时使用套管针两种送管方法的比较研究

摘　　要：目的比较研究外周静脉输液较困难的老年患者应用套管针穿刺时送管的方法．优选出提高老年患者静脉穿刺困难时输液成功率的方法。方法将78例外周静脉穿刺困难的老年患者随机分为观察组和对照组，即观察组患者使用套管针穿刺见回血后操作者右手固定针芯作支撑．由辅助护士将留置针套管慢慢送入血管；对照组则由操作者右手拇指顶住回血腔尾部慢慢将外套管送入血管；两组在整个穿刺过程中操作者的左手在穿刺点下方自始至终绷紧皮肤。结果两种迭管方法的一次穿刺成功率在统计学上有显著的差异（P〈0．01）。结论在外周静脉输液困难的患者应用套管针时．操作者穿刺见回血后右固定针芯作支撑由辅助护士将套管送入血管的方法可以提高此类患者的一次性穿刺成功率．提高患者对护士的满意度．减轻护士给此类患者输液时的心理压力。Objective to compare and research two methods in sending tube when it is difficult to use the catheter to puncture the elderly patient＇peripheral vein, to optimize the method of improving the success rate of infusion when it is difficult to puncture the elderly patient＇ vein. Methods Devide 78 cases of elderly patients with difficult venous puncture into the observation group and the control group. The observation group： the operator uses the catheter to puncture and fixes the needle core as a support with right hand when the operator sees the ruturn of blood during the process of the puncture, then the auxiliary nurse sends indwelling catheter needle into the blood vessel slowly;The control group ：the operator uses right thumb to resist the tail of the flash chamber and sends indwelling catheter needle into the blood vessel slowly,the operator＇s left hand taut skin below the punctue point start to finish. Results A puncture success rate has statistically significant difference between two methods of seeding tube （P 〈 0.01）. Conclusions When the operator uses catheter to puncture the patient＂ peripheral vein that is difficult to puncture ,the method that the operator uses the catheter to puncture and fixes the needle core as a support with right hand when the operator sees the ruturn of blood during the process of the puncture, then the auxiliary nurse sends indwelling catheter needle into the blood vessel slowly,which improves a puncture success rate of these patients ,and Improves the satisfaction of patients to nurses ,and reduces nurses＇ psychological stress of Infusion to such patients.
