滑动加压鹅头钉与改良股骨近端加锁髓内钉在老年股骨粗隆下骨折治疗中的疗效比较

摘　　要：目的:探讨滑动加压鹅头钉(DHS)与改良股骨近端加锁髓内钉(PFNA)在老年股骨粗隆下骨折治疗中的临床应用效果。方法:2008年6月～2010年2月我院共收治60例股骨粗隆下骨折的老年患者,将其随机分为两组,每组各30例,分别给予DHS和PFNA治疗,观察并分析两组的术中情况、优良率及并发症的发生情况。结果:PFNA组的手术时间短于DHS组,术中平均出血量低于DHS组,手术切口的平均长度大于DHS组,两组比较,差异均有统计学意义(t=10.219、18.758、7.364,均P<0.05)。PFNA组的优良率明显高于DHS组,两组比较,差异有统计学意义(χ2=8.63,P<0.05)。髋内翻和内固定松动并发症发生率两组比较,差异均有统计学意义(χ2=8.765、9.132,均P<0.05)。结论:与DHS治疗相比,采用PFNA治疗老年股骨粗隆下骨折的手术时间较短、术中出血量较小、治疗的优良率较高且其并发症的发生率较低,值得在临床中推广应用。Objective： To investigate the clinical effects of DHS and PFNA in the treatment of patients with subtrochanter- ic femoral fracture. Methods： From June 2008 to February 2010, 60 cases of old patients with subtrochanteric femoral fracture definitely diagnosed in our hospital were selected. And put them into two groups as DHS group （30 cases） and PF- NA group （30 eases） at random. The situations in the operation, complication and excellent rate were compared and ana- lyzed in the two groups. Results： There were significant differences about the operation time, average bleeding quantity and the mean length of the Incision in the PFNA group and DHS group （t=10.219, 18.758, 7.364, all P〈0.05）. The excellent rate in the PFNA group was higher than that in the DHS group （χ2=8.63, P〈0.05）. There were significant differences about the complications of coxavara and internal fixation in the two groups （χ2=8.765, 9.132, all P〈0.05）. Conclusion： Compared with DHS, PFNA can be recognized as the prior choice because of shore operation time, little bleeding quantity and the excellent rate in the treatment of old patients with subtrocbanteric femoral fracture. It＇s worthy of clinical application.
