摘　　要：目的了解传统隔离策略下传染病医院肝病内科医院感染发病基本特征。方法前瞻性调查2007年1月-2008年12月某三级甲等传染病专科教学医院肝病内科医院感染发生率。结果共调查住院患者4343例,医院感染394例,发病密度为3.030‰,发病率为9.1%;医院感染发病例次数457例次,例次发病密度为3.515‰,例次发病率为10.5%;常见医院感染发病部位依次为:原发性腹膜炎、血流感染、上呼吸道感染和下呼吸道感染,分别占22.5%、19.9%、19.7%及16.6%;医院内下呼吸道感染死亡和血流感染死亡最多;重型肝炎病区的医院感染发生强度最高,为5.02‰,其中医院内血流感染、下呼吸道感染例次发病强度分别为1.65‰、1.08‰,为全院之最。结论传统隔离策略下传染病医院肝病内科医院感染的分布以呼吸道感染、原发性细菌性腹膜炎和血流感染为主,医院内下呼吸道感染和血流感染的转归最为严重。OBJECTIVE To continuously evaluate nosocomial infections in gastroenterological units of an infectious disease hospital.METHODS The incidence of nosocomial infections（NIs） in gastroenterological units from Jan 2007.to Dec 2008.was prospectively surveyed.RESULTS All 4343 cases were involved during the surveillance,of which 394 patiend acquired NIs.And the average incidence of NIs was 3.030 cases per 1000 admitted patient-day.457 case-times attacked,the case-times incidence of NIs was 3.515 case-times per 1000 admitted patient-days.The more frequent NI sites were spontaneous bacterial peritonitis（SBP,23%）,blood stream infection（BSI,20%）,upper respiratory tract infection（URTIs,20%） and lower respiratory tract infection（LRTIs,17%）,which accounted to 80% of NIs.Nosocomial LRTIs or BSI were the two leading causes of death assoliated with Nis.The NI incidence in a unit mainly admitting liver failure was the highest 5.02‰,in which BSI 1.65 and 1.08‰ were the highest also.CONCLUSION Respiratory tract infection,SBP and BSI are the major NI of infectious disease hospital on the Chinese traditional infectious disease isolation and prevention strategy.The mortality is higher among patients with nosocomial LRTIs and BSI than other NIs.
