性激素与绝经女性尿路感染的临床分析

摘　　要：目的探讨性激素水平改变与女性尿路感染的关系。方法选取女性尿路感染患者99例,其中复杂性尿路感染(CUTI)者75例,非复杂性尿路感染者24例;将患者分为绝经组(75例)和非绝经组(24例);采用电化学发光法测定性激素水平(包括血浆雌二醇、睾酮、孕酮、促卵泡刺激素、黄体生成素及垂体泌乳素水平),并根据测定结果分析血性激素水平改变与女性尿路感染的相关性。结果两组患者的血浆雌二醇、睾酮、孕酮、促卵泡刺激素、黄体生成素及垂体泌乳素水平差异无统计学意义;而CUTI组雌二醇与睾酮的比值为0.160±0.178,显著低于非CUTI组的0.588±1.02,差异有统计学意义(P<0.01);绝经组雌二醇、促卵泡刺激素、黄体生成素水平显著高于非绝经组(P<0.01);雌二醇/睾酮、孕酮均显著低于非绝经组(P<0.01)。结论血浆雌二醇与睾酮的比值与女性复杂性尿路感染的发生可能有关;绝经后女性血浆雌二醇、睾酮、雌二醇/睾酮、孕酮、促卵泡刺激素、黄体生成素水平发生了明显的变化,更容易发生尿路感染。OBJECTIVE To analyze the relationship between sex hormone levels and women with urinary tract infections.METHODS A total of 99 cases of female patients with urinary tract infection were enrolled.The patients′ average age was 59.4 ＋ 14.3.Seventy-five patients among them had complexity urinary tract infection（CUTI）.The patients were divided into postmenopausal group（75 cases） and the non-postmenopausal group（24 cases）.All sex hormone levels（consist of E2,TTE,PGN,FSH,LH,PRL） were determined by electrochemiluminescence.According to test results,the correlation of the change of plasma sex hormone levels and the occurrence of the urinary tract infection in women were analyzed.RESULTS The plasma oestracliol/testosterone ratios in CUTI patients was much lower than UTI（P0.01）,and the oestracliol/testosterone ratios in CUTI patients were 0.160±0.178,which were significantly lower than 0.588±1.02 in non-postmenopausal group（P0.01）.and there were significant differences between the postmenopausal and the non-postmenopausal in the levels of oestracliol,oestracliol/testosterone ratios,progestron,flitropin and luteinizing hormone（P0.01）.CONCLUSIONS Plasma oestracliol/testosterone ratio may correlate with CUTI in women.The plasma oestracliol,testosterone,oestracliol/testosterone ratio,progestron,flitropin,luteinizing hormone levels in postmenopausal women change visibly,which can cause UTI more easily.
