实时超声引导下经皮肾脏穿刺活检技术的应用及临床诊断价值

摘　　要：目的:观察实时超声引导下穿刺肾脏活组织取材的成功率、并发症的发生,探讨此技术的临床诊断价值。方法:42例肾疾病患者在超声引导下,采用自动活检针快速肾脏活体组织取材,送病理检查,增常规行光镜、电镜及免疫荧光检查。结果:除1例因取材不足未能满足诊断要求外,41例均取得正确临床病理诊断,占97.6%。3例术后出现并发症,其中肉眼血尿2例,肾周血肿1例,经对症治疗后均好转。结论:实时超声引导下经皮肾穿刺活检操作时间短,方法简单,定位精确,穿刺命中率高,并发症少,是一种安全有效的方法,具有较高的临床应用价值。Objective：To investigate the successful rate and complications of real-time ultrasonography-guided percutaneous biopsy of kidney,and to evaluate the clinical value of this technique.Methods：42 patients with nephritic diseases underwent real-time ultrasonography guided percutaneous biopsy of kidney with an automatic biopsy gun.The collected specimens were sent to pathology department and examined with routine light-microscopy,electronic microscopy and immunofluorescence assay.Results：Apart from 1 patient could no fulfilled the diagnosis requirement due to too little the amount of specimen,the successful rate of renal biopsy was 97.6%.3 cases（7%） had postoperative complication,including gross hematuria（2 patients） and perirenal hematoma（1 patient）,all recovered after proper treatement.Conclusion：Real-time ultrasonography guided percutaneous renal biopsy is a simple,accurate,time-saving technique,with high successful rate and less complications.
