新生儿乙肝母婴阻断免疫策略及相关因素研究

摘　　要：[目的]探讨不同免疫策略对乙肝母婴阻断成功率的影响,为制定乙肝防控策略提供科学依据。[方法]从医院收集2007~2009年HBsAg阳性产妇及其新生儿基本资料,采集儿童静脉血2ml,ELISA法检测乙肝表面抗原和表面抗体。将乙肝母婴阻断的免疫策略分成A组（孕晚期接种HBIG＋儿童出生后接种HBIG＋乙肝疫苗）、B组（儿童出生后接种乙肝免疫球蛋白＋乙肝疫苗）、C组（乙肝疫苗）3组,比较不同免疫策略的乙肝母婴阻断成功率。[结果]不同免疫策略乙肝母婴阻断成功率不同,A、B两组无差别,但均高于C组（χ2=6.64,P=0.01）。在调查的312名母亲HBsAg阳性的儿童中,乙肝母婴阻断成功率为95.53%（299/313）,影响乙肝母婴阻断成功的因素包括分娩方式和母亲是否为乙肝大三阳。[结论]分娩时尽量选择剖宫产,出生后联合接种乙肝免疫球蛋白和乙肝疫苗的策略会提高乙肝母婴阻断成功率。[Objective]To study the influential factors of immunization strategies in preventing mother-to-child transmission of（PMTCT）hepatitis B and to provide scientific basis of prevention and control strategies for the development of hepatitis B.[Methods]Collected basic information of HBsAg-positive mothers and their newborns,from the hospital from 2007 to 2009,and 2ml navel blood were collected from the infants to detect hepatitis B virus infection by ELISA. Three kinds of immunization strategies of PMTCT were divided into A group（third trimester of pregnancy inoculated with HBIG ＋ vaccination HBIG ＋ HB vaccine）,B group（only children born after the inoculation of hepatitis B immune globulin ＋ hepatitis B vaccine）,C group（only hepatitis B vaccine）,the compared the PMTCT success rate among different groups.[Results]The PMTCT success rate varied among three groups（χ2 = 6.64,P = 0.01）. The success rate of hepatitis B in PMTCT was 95.53%（299/313）,the influential factors for success of hepatitis B PMTCT included the mode of delivery and whether the mother’s HBeAg was positive.[Conclusion]Choosing cesarean delivery,immunization globulin and hepatitis B vaccine strategies will improve the success rate of hepatitis B PMTCT.
