上海市杨浦区居民伤害流行病学调查研究

摘　　要：目的了解上海市杨浦区居民伤害发生的现状,评价其社会经济损失,为进一步开展预防提供依据。方法采用多阶段随机抽样的方法,对4个街道48个居委的29826名居民进行了伤害状况的回顾性调查。结果伤害的发生率为10.53%,标化发生率为8.12%,女性高于男性,位居前5位的伤害依次是跌倒、割刺伤、交通伤、扭伤、碰撞挤压伤。0～14岁以跌倒、碰撞挤压伤为主;15～34岁以交通伤、扭伤为主,35～59岁以割刺伤、交通伤为主,≥60岁以跌倒、割刺伤为主。致残率0.83%,残疾率87.17/10万,致死率为0.13%,死亡率为13.41/10万,平均每例死亡的潜在寿命损失年数21年,潜在工作损失年为11年。每例直接经济损失2802元,人均住院3.7d,因伤休息26d,需他人看护14d。结论伤害在杨浦区居民中常见、多发,造成的后遗伤残、潜在寿命损失和经济损失均较大。Objectives To investigate the current status of injury and to estimate its socio-economic burden for injury prevention. Methods A retrospective study on injury was conducted with multi-stage cluster random sampling among 29 826 people from 48 urban residential committees. Results Incidence rate of Injury was 10.53%, with the standardized rate of 8.12%, and the rate was higher in women than in men. The top five causes of injuries were falls, injury by sharp articles, traffic, wrench and collision. Falls and collision occurred mostly in population with 0-14 yrs. Traffic and wrench occurred mostly in 15-34 yrs people. Injury by sharp articles and traffic occurred mostly in 35-59 yrs residents. Falls and injury by sharp articles occurred mostly in people with ≥60 years. Disability rate caused by injury was 87.17/100 000. Mortality rate was 13.41/100 000. In average, there was 21 years of potential life lost and 11 work years of potential life. Direct economic loss cost 2 802 Yuan RMB. The rested days was 26 days and was cared for 14 days for each case with injury. Conclusion Injury was a common and frequently happened incident among the residents, induced heavy socio-economic burden and became an important public health problem harmful to the people＇ s health. The injury prevention should be enhanced.
