摘　　要：目的探讨2型糖尿病(T2DM)患者腰身指数(WHtR)与胰岛功能的相关性。方法 T2DM患者179例,以WHtR0.5为切点分为非肥胖组和肥胖组,比较两组间的稳态模型胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMA-IS)、胰岛素曲线下面积(AUCINS)和血糖曲线下面积(AUCPG),并行Pearson相关分析和多元逐步回归分析。结果肥胖组较非肥胖组ln(AUCINS)、ln(HOMA-IR)和ln(HOMA-IS)升高,差别具有统计学意义(P<0.05);WHtR与ln(AUCINS)、ln(HOMA-IR)和ln(HOMA-IS)呈正相关,r值分别为0.392、0.359、0.414,P<0.01;多元逐步回归分析显示,ln(HOMA-IS)是WHtR的独立影响因素(P<0.01)。结论 WHtR是胰岛素抵抗和高胰岛素血症良好的预测因子。Objective To investigate the relationship between waist-height ratio （WHtR） and insulin function in patients with type 2 diabetes mellitus （T2DM）. Methods With the cut point of WHtR at 0.5, 179 of T2DM patients were divided into nonobesity group and obesity group. The difference in insulin resistance index （HOMA-IR）, insulin secretion index （HOMA-IS）, area under curve of insulin （AUC INS） and area under curve of blood glucose （AUC PC）were compared between two groups. The Pearson correlation analysis and multivariate stepwise regression analysis were then made. Results The ln（AUC INS）, ln（HOMA-IR）, In （HOMA-IS） in obesity group were all higher than those in non-obesity group （P〈0.05）. WHtR was positively correlated with ln（AUC INS） （r=392, P〈0.01）, ln（HOMA-IR） （r=359, P〈0.01） and ln（HOMA-IS） （r=414, P〈0.01）. Multivariate stepwise regression analysis revealed that ln（AUC INS） was one of main influence factors on WHtR （P〈0.01）. Conclusion WHtR maybe a good predict factor for insulin resistance and hyperinsulinemia.
