环肺静脉电隔离消融与药物治疗心房颤动对患者生活质量影响的研究

摘　　要：目的评价环肺静脉电隔离消融与药物治疗心房颤动对患者生活质量的影响。方法 113例心房颤动患者,分别采用环肺静脉电隔离消融与抗心律失常药物治疗,其中消融组58例,药物组55例,进行随访,6个月后采用SF-36量表问卷调查方式对两组患者生活质量进行评价。结果平均随访(14.7±5.3)月,消融组维持窦性心律者33例,成功率56.9%,药物组维持窦性心律者14例,成功率25.5%。两组患者治疗6个月后SF-36量表中除肌体疼痛差异无统计学意义外,躯体功能、躯体角色、总的健康状况、活力、社会功能、情感角色和心理健康,消融组较药物组均有明显改善(P<0.05)。结论环肺静脉电隔离消融较抗心律失常药物治疗房颤能明显提高患者的生活质量。Objective To evaluate different impacts of circumferential pulmonary vein isolation and antiarrhythmic drug therapy on quality of life （QOL） in patients with atrial fibrillation （AF）. Methods 113 patients with atrial fibrillation were divided into two groups, radiofrequency catheter ablation group （n=58） and antiarrhythnfic drug therapy group （n=55）. Short-Form 36 （SF-36） was used to evaluate the QOL of these patients. The data were collected before therapy, and evaluated in 6 months after therapy. Results After a mean follow-up of （14.7±5.3） months, 33 patients were free from AF in radiofrequency catheter ablation group, the success rate was 56.9%; and 14 patients were free from AF in drug treatment group, the rate was 25.5%. There were significant improvements in QOL in radiofrequency catheter ablation group （except for body pain）. Conclusion Circumferential pulmonary vein isolation can improve quality of life in patients with atrial fibrillation than antiarrhythmic drugs does.
