摘 要:目的观察糖络宁治疗糖尿病周围神经病变(DPN)的临床疗效并对其作用机制进行初步探讨。方法选择符合标准的DPN患者62例,随机分为治疗组32例和对照组30例。在糖尿病基础治疗的同时,治疗组口服糖络宁,对照组口服弥可保,疗程均为2个月。观察治疗前后患者临床症状、神经病变积分(MDNS)及神经传导速度的改善情况,并观察治疗前后血清丙二醛(MDA)含量、超氧化物歧化酶(SOD)活性水平及总抗氧化能力(TAOC)的变化。结果治疗组总有效率为93.8%(30/32),明显优于对照组的70.0%(P<0.01)。2组治疗后临床症状积分、MDNS积分均明显下降,且治疗组明显低于对照组(P<0.01)。2组治疗后神经传导速度均明显改善,治疗组优于对照组(P<0.05)。2组治疗后血清SOD活性及TAOC水平明显升高,MDA水平明显下降,TAOC/MDA明显升高,TAOC与MDA呈负相关(r=-0.206,P<0.01),且治疗组优于对照组(P<0.01)。在治疗过程中未见明显不良反应。结论糖络宁治疗DPN疗效确切,其作用机制可能与抗氧化应激有关。Objective To observe the therapeutic effects and antioxidative effect of Tangluoning on diabetic peripheral neuropathy(DPN).Methods Sixty-two DPN patients met the diagnostic criteria were randomly divided into the treatment group(32 patients) and control group(30 patients).On the basis of routine treatment,the treatment group was given Tangluoning,and the control group was given Methycobal.The changes of symptoms,Michigan diabetic neuropathy intergrals(MDNS) and nerve conductive velocity of both groups were observed.The content of malonaldehyde(MDA),the activity of superoxide dismutase(SOD) and the total antioxidant capacity(TAOC) in serum was measured.Results The total effective rate of treatment group was 93.8%(30/32),which was obviously superior to that of control group 70.0%(P〈0.01).The symptom score and MDNS were decreased obviously,the effect of treatment group was superior to that of control group(P〈0.01).There were significant differences in the improvement of nerve conductive velocity between the two groups(P〈0.05).After treatment,MDA content decreased significantly,and the level of SOD,TAOC and the ratio TAOC/MDA increased significantly in both groups,TAOC level was negative correlated with MDA level(r=-0.206,P〈0.01).The effect of treatment group was superior to that of control group(P〈0.01).There were no obvious side effects during the treatment.Conclusion Tangluoning could obviously improve therapeutic effect on DPN by protecting against oxidative stress.
