摘 要:目的:了解早期干预对早产儿智力、运动发育及脑瘫发生率的影响,探讨降低脑瘫发生率的有效方法。方法:对136名早产儿按家长意愿分为早期干预组61例和对照组75例,干预组在纠正胎龄满40周后做20项新生儿行为神经测评,评分<35分者建议在医院进行干预治疗(包括运动疗法和物理治疗,配合运用导平、脑循环治疗仪等);评分≥35分者给家长发放指导手册,指导家长在家中进行干预训练;在随访中根据神经测评情况调整干预方法,定期对早产儿进行全面早期教育的基础上重点进行运动训练。对照组在本院儿保门诊接受常规育儿指导。所有婴儿定期进行体格检查和神经运动检查。结果:两组男女比例、平均胎龄和出生体重、分娩方式和纠正胎龄40周新生儿行为神经测查评分差异均无统计学意义(P>0.05),说明两组有可比性。在8月龄、12月龄时CDCC智测结果得分干预组均较对照组高,两组差异有统计学意义(P<0.05)。12月龄干预组有1例脑瘫,对照组有7例,两组差异有统计学意义(P<0.05)。结论:早产儿早期干预可促进早产儿智力和运动发育,减少脑瘫的发生率。Objective:To understand the effect of early intervention on intelligence development,motor development and incidence of cerebral palsy in premature infants,explore an effective method to reduce the incidence of cerebral palsy.Methods:136 premature infants were divided into early intervention group(61 premature infants) and control group(75 premature infants) according to the intentions of their parents.When the corrective gestational age reached 40 weeks,the premature infants in early intervention group were assessed by neonatal behavioral neurological assessment(NBNA)(20 items),it was recommended that the premature infants with NBNA score35 received intervention in the hospital(including kinesitherapy,physiotherapy,therapy introduction and cerebral circulation therapeutic equipment);while the premature infants with NBNA score35 received intervention at home through their parents(directed by a guide book);the intervention method was regulated according to NBNA score in the course of follow-up,exercise training was conducted on the basis of regular,comprehensive and early education.The premature infants in control group received routine child-rearing guldance in outpatient department of child health care in the hospital.All the premature infants received physical examination and neuromotor examination regularly.Results:There was no significant difference in sex ratio,average fetal age and birth weight,delivery mode and NBNA score when the corrective gestational age reached 40 weeks between the two groups(P0.05).The scores of CDCC at 8 and 12 months in intervention group were significantly higher than those in control group(P0.05).At 12 months,1 premature infant was found with cerebral palsy in intervention group and 7 premature infants were found with cerebral palsy in control group,there was significant difference between the two groups(P0.05).Conclusion:Early intervention can promote the intelligence development and motor development of premature infants,reduce the inc
