经阴道三维超声成像与X线子宫输卵管造影诊断先天性子宫畸形的对比研究

摘　　要：目的:探讨经阴道三维成像技术与传统X线子宫输卵管造影诊断先天性子宫畸形的诊断价值,评价两种影像学检查诊断先天性子宫畸形的优缺点。方法:对可疑子宫畸形的47例患者分别行X线子宫输卵管造影术和经阴道三维超声检查,比较两种方法诊断先天性子宫畸形的诊断结果,并与宫腔镜或腹腔镜结果作对照。结果:经阴道三维超声诊断先天性子宫畸形的敏感性与特异性可达100%;而X线子宫输卵管造影对先天性子宫畸形的分型具有一定的局限性,尤其在区分双角子宫或是纵隔子宫,单角子宫是否合并残角子宫的诊断上难以做出准确判断。结论:经阴道三维超声检查能显示3个相互垂直的平面,从多角度、多平面分析图像,尤其是可以通过子宫冠状面的三维成像直观地显示宫底外观形态及宫腔内部结构,弥补了X线子宫输卵管造影诊断子宫畸形的不足,为临床医师提供更加准确的诊断依据。Objective：To explore the values of transvaginal three-dimension echocardiography and hysterosalpingography under X-ray in diagnosis of congenital uterine deformity,evaluate the advantages and disadvantages of the two imaging examination methods.Methods：47 cases suspected of uterine deformity received hysterosalpingography under X-ray and transvaginal three-dimension echocardiography,respectively,the diagnostic results of the two methods were compared,then the results were compared with those of hysteroscope and laparoscope.Results：The sensitivity and specificity of transvaginal three-dimension echocardiography in diagnosis of congenital uterine deformity both were 100%,but hysterosalpingography under X-ray had a certain limitation in diagnosis of types of congenital uterine deformity,especially for the differentiation of uterus bicornis and separate uterus,uterus unicornis combining with rudimentary horn uterus or not.Conclusion：Transvaginal three-dimension echocardiography can display three planes which plumb each other,analyze the images in more angles and more planes,especially can show the appearance and shape of uterine fundus,the inner structure of uterine cavity by three dimensional imaging on coronal plane of uterus,which makes up for the deficiency of hysterosalpingography under X-ray in diagnosis of congenital uterine deformity and provides more accurate diagnostic criterion for clinicians.
