人胎盘凝血活酶试剂的标定及其临床应用

摘　　要：目的：标定自制人胎盘凝血活酶并应用于临床病人测定。方法：根据ＷＨＯ 参比方法，用已知国际敏感指数（ＩＳＩ） 的凝血活酶标定自制人胎盘凝血活酶；用进口试剂和自制品测定６０例口服华法令抗凝治疗病人和４０ 例肝病患者的ＰＴ，求得ＰＴＲ 和ＩＮＲ 并进行比较。结果：自制品ＩＳＩ为１ ．３９ ，与进口品测定同一标本所得ＩＮＲ比较的回归方程为Ｙ＝ ０ ．９８３６Ｘ＋ ０ ．０６２４９ ，两种试剂测定６０ 例抗凝治疗病人所得ＰＴ、ＰＴＲ有明显差异而ＩＮＲ 无差异。用人胎盘凝血活酶测得４０ 例健康人ＩＮＲ为０ ．８０ ～１ ．２４ ，６０ 例机械性换瓣病人ＩＮＲ 值为１ ．２６ ～２ ．９２ 。结论：自制人胎盘凝血活酶ＩＳＩ值的标定是准确的，可以应用于临床。Objective:To calibrate the self made human placenta thromboplast in and to use it to clinical detection Methods:Calibration of the self made thromboplastin was performed according to the method of ISI detection recommended by the WHO.The PT.PTR.and INR of 60 patients treated with oral anticoagulants and 40 patients with liver disorders were determined with the two kinds of thromboplastin reagents.Results:The ISI of the human placenta thromboplastin was 1.39.The PT and PTR determined with two kinds of thromboplastin reagents were different significantly,but the INR were very similar(Y=0.9836X+0.0629),with the human placenta thromboplastin used to determine,the INR of 60 patients treated with oral anticoagulants were 1.26～2.92,the INR of 40 healthy human plasmas were 0.80 ～1.24.Conclusion:The ISI of the self made human placenta thromboplastin is accurate,and this reagent can be used in the clinical fields.
