后腹腔镜肾癌根治切除术78例临床分析

摘　　要：目的总结后腹腔镜肾癌根治术的临床经验。方法回顾性分析我院自2005年3月～2010年5月行后腹腔镜进行肾癌根治术78例肾癌患者的临床资料,总结手术方法及观察治疗效果。结果本组78例患者进行后腹腔镜肾癌根治术均成功,手术成功率为100%,手术时间为55～145min,平均105min;术中出血量为80～1500mL,平均150mL;1例左肾静脉损伤,但未改开放手术,6例腹膜损伤,1例膈肌损伤,2例皮下气肿,1例术后出现肾周围组织粘连。术后肠功能恢复时间为7～26h,平均10.2h;住院时间为3～11d,平均5.6d。随访6个月至3年,无局部复发或远处转移,无切口或操作孔肿瘤种植。结论后腹腔镜肾癌根治术具有安全性高、创伤小、术后恢复快、并发症少等优点,是一种安全、有效的治疗方法。【Objective】 To summarize our experiences of retroperitoneoscopic radical nephrectomy in treating localized renal cell carcinoma.【Methods】 Between March 2005 and May 2010,78 cases of retroperitoneoscopic radical nephrectomy were performed for localized renal cell carcinoma.We reviewed the clinical data and oncologic outcome.【Results】 Of all the cases none needs to transfer to open surgery.The mean operative time was 105 min（55 to 145）,the mean blood loss was 150 mL（80 to 1500 mL）.The operative complications included peritoneal injury（6 cases）,diaphragmatic muscle injury（1 case）,subcutaneous emphysema（2 cases）,and renal vein injury（one case without conversion to open surgery）.The mean recovery time of the bowel function was 10.2 hours（7-26 hours）,and the mean postoperative stay was 5.6（3 to 11） days.During the follow-up time from 6 months to 3 years,no local recurrence or metastasis was found.No tumor plantation at the trocar site.【Conclusion】 Retroperitoneoscopic radical nephrectomy is safe and effective.
