摘  要:目的描述腰椎间盘周围血管的的影像学、薄层断面解剖学特征,探明穿刺出血并发症的解剖学基础。方法对53例正常腰椎间盘的CT图像、40例正常腰椎间盘的MRI图像和2例中国数字化可视人体数据腰段薄层图像中的腰椎间盘周围血管及其与后外侧穿刺入路关系进行观察和测量。结果获得腰椎间盘周围血管及其与后外侧穿刺入路关系的影像学、薄层断面解剖学特征和相关测量参数。发现对腰椎间盘周围血管的显示以薄层断面最优,MRI次之,CT最差。虽然后外侧穿刺入路安全三角是比较安全的区域,但有腰动脉脊支、椎间静脉和椎内静脉等血管结构通过。结论经过对腰椎间盘周围血管的影像断面和薄层断面图像进行对照观察,发现后者能提高腰椎间盘周围血管的细微结构CT及MRI的影像确认率。薄层断面解剖进一步证实腰动脉脊支、椎间静脉和椎内静脉是穿刺出血的解剖学基础。Objective To describe the imaging and thin section features of the peripheral blood vessel in lumbar vertebral disc and ascertain anatomical basis of puncture bleeding complications. Methods Lumbar peripheral vascular and with the posterolateral puncture approaches relations were observed and measured in 53 patients with normal CT images, 40 cases of normal MRI images and 2 cases of Chinese digital visible human data. Results Thin section anatomic features and correlative measuring parameters in lumbar peripheral vascular and with the posterolateral puncture approaches relationship were obtained. The lumbar intervertebral disc peripheral vascular displayed best with thin section and worst with CT. The posterolateral puncture into the road safety triangle was safer areas, but there were between vertebra artery vein, lumbar vertebral vein ridge supported and within such vascular structures through. The safety triangle posterolateral puncture road wass safer areas, but there were the vascular structures of rami spinales of lumbar artery, intervertebral vein and internal vertebral plexus through within. Conclusion The thin section image can improve the confirm rate of subtle structure of lumbar peripheral vessels. Thin section anatomy further proved rami spinales of lumbar artery, intervertebral vein and internal vertebral plexus is the anatomical basis of puncture bleeding.
