肺减容在食管癌合并重度COPD患者手术中的临床应用

摘　　要：目的探讨肺减容术(LVRS)在治疗食管癌合并重度COPD患者时的手术方法及疗效。方法 35例食管癌合并重度COPD患者行食管癌切除同期一侧LVRS。术前、术后肺功能、血气分析对比。结果所有患者均顺利渡过围手术期。肺功能和动脉血气明显改善。结论 LVRS为部分食管癌合并重度COPD患者提供了新的治疗方法,不但保证术后围手术期的平稳过渡,而且通过改善肺功能提高患者生活质量。Objective To discuss the operative methods and the clinical effects of lung volume reduction surgery（LVRS） in tumor resection of esophageal carcinoma combined with severe COPD.Methods Esophageal carcinoma resection were performed in 35 cases with pulmonary diseases followed by the same side LVRS.The pulmonary functions and blood gas analysis before and after operation were compared.Results All patients survived the perioperative stage safely.FEV1,FEV1/FVC,DLCO,RV,PaO2,and PaCO2 were significantly improved.Conclusion LVRS might be a new operative method for some patients with severe COPD combined with Esophageal carcinoma,and might also provide conditions for patients to stably survive the perioperative period and for the improvement of postoperative pulmonary functions.
