APOA1、PA、CRP及血小板参数在儿童全身炎症反应综合征中的价值

摘　　要：目的:探讨载脂蛋白A1(APOA1)、前白蛋白(PA)、血清C-反应蛋白(CRP)及血小板参数水平的变化在SIRS治疗及预后中的价值。方法:将86例全身炎症反应综合征(SIRS)患儿按是否发生多器官功能障碍将其分为A组39例和B组47例,对照组选取40例健康患儿,用免疫透射比浊法测定APOA1,免疫透射比浊终点法测定PA,胶乳增强免疫透射比浊法测定CRP、电阻抗法+流式细胞术测定PLT、MPV及PDW,将各组测定值进行比较。结果:①A组与B组比较,APOA1、PA及PLT明显下降,CRP、MPV及PDW明显升高(P<0.05),A组与对照组比较,APOA1、PA及PLT下降与CRP、MPV及PDW升高更明显(P<0.01);②B组与对照组比较APOA1、PA及PLT下降与CRP、MPV及PDW升高明显(P<0.05);③治疗3天后复查,A组及对照组各项指标变化不明显,与治疗前比较差异无统计学意义(P>0.05),B组APOA1、PA及PLT明显升高,CRP、MPV及PDW明显降低,与治疗前比较差异有统计学意义(P<0.05);④追踪随访,A组死亡12例,死亡率30.77%,B组死亡1例,死亡率2.13%,对照组无死亡病例。结论:动态观察APOA1、PA、CRP及血小板参数的变化对SIRS的疗效及预后具有指导意义。Objective：To explore the values of changes of apolipoprotein A1（APOA1）,prealbumin（PA）,C-reactive protein（CRP） and platelet parameters in treatment and prognosis of systemic inflammatory response syndrome（SIRS）.Methods：86 children with SIRS were divided into A group（39 children） and B group（47 children） according to combining with multiple organ dysfunction or not,40 healthy children were selected as control group,immune turbidimetric method was used to detect APO A1,immune turbidimetric end point method was used to detect PA,latex enhanced immune turbidimetric method was used to detect CRP,electrical impedance method and flow cytometry were used to detect PLT,MPV and PDW;then the indexes in the three groups were compared.Results：Compared with B group,APO A1,PA,and PLT in A group decreased significantly,CRP,MPV and PDW increased significantly（P〈0.05）;compared with control group,the decreases of APO A1,PA,and PLT and the increases of CRP,MPV and PDW in A group were more obviously（P〈0.01）;compared with control group,the decreases of APO A1,PA,and PLT and the increases of CRP,MPV and PDW in B group were obviously（P〈0.05）;after treated for 3 days,the changes of indexes in A group and control group were not obviously,and there was no significant difference before and after treatment（P〈0.05）;the death rates in A group and B group were 30.77%（12 children） and 2.13%（1 child）,respectively,no children died in control group.Conclusion：Dynamic observation on the changes of APO A1,PA,CRP and platelet parameters has directive significance for efficacy and prognosis of SIRS.
