摘　　要：目的探讨AECOPD患者呼出气冷凝液中pH变化的临床意义。方法选择2005年7月～2010年10月笔者所在医院就诊的AECOPD患者115例,另选同期健康体检者100例。比较AECOPD治疗前后EBCpH的变化及与健康对照组EBCpH比较,探讨AECOPD治疗前EBCpH与肺功能(FEV1、FEV1/FVC、FEV1%预计值)的相关性。结果 AECOPD组治疗前后比较,pH明显升高,差异具有统计学意义(P<0.05);治疗前后与对照组比较,pH明显降低,差异有统计学意义(P<0.05)。但AECOPD的EBC中pH与肺功能无明显相关性(P>0.05)。结论 AECOPD患者EBCpH显著降低,气道酸化明显,EBC中pH可作为监测气道酸碱度和COPD急性发作的有效指标之一,但不能反映肺功能和病情严重情况。Objective To explore the clinical significance of pH in expired breath condensate with acute exacerbation chronic obstructive pulmonary diseasebefore and after treatment.Methods 115 AECOPD patient（sexperimental group）and 100 person（scontrol group）with normal PE results were chosen from the author’s hospital between Jul.2005 and Oct.2010.The changes of EBC pH were compared between the AECOPD patients before and after treatment as well as the control group.Moreover,the relationship in AECOPD group between EBC pH and lung function（FEVl、FEVl/FVC、FEVl% predicted value）was assessed.Results The EBC pH value in the AECOPD patients after treatment was significantly higher compared to before treatment（P 〈 0.05）but significantly lower than the control group（P〈0.05）.EBC pH values manifested no correlation between FEV1、FEV1/FVC and FEV1% predicted valu（eP 〉0.05） in AECOPD patients before treatment.Conclusion We concluded that pH decreased obviously in the EBC of patients with AECOPD,which means an obvious process of airway acidification.The measurement of the EBC pH may be useful in screening and monitoring airway acidification and airway inflammation of patient with AECOPD,and the change of pathogenetic condition in COPD.Nevertheless,it could not reflect the change of lung function and severity of disease.
