摘  要:目的了解新生儿缺氧缺血性脑病(HIE)中红细胞膜Na+-K+-ATP酶活性、红细胞膜流动性变化及其临床意义。方法选择新生儿科住院的中、重度HIE患儿65例,于急性期、恢复期采血测定红细胞膜Na+K+-ATP酶活性、红细胞膜流动性,并于同期行新生儿神经行为测定(NBNA)评分,对照组选择早期肺炎、咽下综合征、食管闭锁新生儿30例行相同测定,探讨HIE患儿红细胞膜Na+-K+-ATP酶活性、红细胞膜流动性、NBNA的相关关系。采用SPSS13.0统计软件进行统计学分析,组间比较采用单因素方差分析,相关性分析采用直线相关分析,以P<0.05为差异具有统计学意义。结果①中、重度HIE急性期患儿的红细胞膜Na+-K+-ATP酶活性、红细胞膜流动性均明显低于对照组(P<0.01),其中以重度组最低,亦显著低于同期中度组(P<0.05)。恢复期时中度组红细胞膜Na+-K+-ATP酶活性、红细胞膜流动性较同组急性期明显升高(P<0.01),且基本达到对照组水平(P>0.05);②HIE组急性期红细胞膜Na+-K+-ATP酶活性与NBNA评分呈正相关(r=0.507,P<0.05);HIE组恢复期红细胞膜Na+-K+-ATP酶活性与NBNA评分呈正相关(r=0.268,P<0.05);③HIE组急性期红细胞膜流动性与NBNA评分呈正相关(r=0.572,P<0.05);HIE组恢复期红细胞膜流动性与NBNA评分呈正相关(r=0.57,P<0.05)。结论临床检测红细胞膜Na+-K+-ATP酶活性、膜流动性有利于病情早期判断及预后估计。Objective To investigate the alteration and significance of Na+-K+-ATPase activity and erythrocyte membrane fluidity in newborns with hypoxic ischemic encephalopathy (HIE).Methods Moderate and severe HIE newborns (n=65,each) hospitalized in our department of neonatology were selected and collected for blood samples at acute and recovery stages for determination of Na+-K+-ATPase activity and erythrocyte membrane fluidity,and meanwhile evaluated with NBNA scoring.The same assessments were completed in 30 newborns with early pneumonia,deglutition syndrome or esophageal atresia (control group).The association between the Na+-K+-ATPase activity,erythrocyte membrane fluidity and NBNA scores was then studied in HIE newborns.SPSS 13.0 statistical software was used in statistic analysis,including univariate analysis for inter-group comparison and linear correlation analysis for correlation analysis.Results ① During acute stage,newborns with moderate and severe HIE had significantly lower Na+-K+-ATPase activity and erythrocyte membrane fluidity as compared with the controls (P〈0.01).Notably,these parameters were even lower in neonates with severe HIE as compared with those with moderate HIE at the same period (P〈0.05).The Na+-K+-ATPase activity and erythrocyte membrane fluidity during recovery period in moderate HIE group were significantly elevated as compared with those in acute period (P〈0.01),basically approaching the level of control group (P〈0.05).② Linear correlation analysis showed that the Na+-K+-ATPase activity of the erythrocyte membrane was positively correlated with NBNA scores in HIE group in acute period (r=0.507,P〈0.05) as well as with those in recovery period (r=0.268,P〈0.05).③ Linear correlation analysis also showed a positive correlation between the erythrocyte membrane fluidity and NBNA scores in HIE group during acute period (r=0.572,P〈0.05) and during recovery period (r=0.57,P〈0.05).Conclusion The Na+-K+-ATPa
