老年髋部骨折局部胰岛素样生长因子及其结合蛋白表达的研究

摘  要:目的研究不同类型老年女性髋部骨折中局部胰岛素样生长因子1(IGF-1)及血清胰岛素样生长因子结合蛋白3、4、5(IGF-BP-3、4、5)表达差异。方法选取年龄大于65岁老年女性髋部骨折46例,比较局部IGF-1及IGFBP-3、4、5蛋白表达差异。结果股骨粗隆间骨折组粗隆区BMD较股骨颈骨折组低(P<0.001),局部蛋白表达只有IGFBP-4存在差异(P=0.004)。结论局部IGF-BP-4表达的差异可能是导致不同类型老年髋部骨折的病理机制之一,但由于IGF-1及其结合蛋白调节机制相当复杂,对于IGF-1及IGFBPs表达机制同骨折类型的关系尚需大量的实验研究进一步证实。Objective To investigate differences of local insulin-like growth factor-1 (IGF-1) and IGF-binding protein-3,4,5 (IGFBP- 3,4,5)expressions, in female senile hip fracture of two types. Methods The differences of local IGF-1 and IGFBP-3,4,5 expressions in selected female sufferers of senile hip fracture aged over 65 were examined and compared. Results Among 46 cases, BMD, and local protein expression of IGFBP-4 in the intertrochanteric fracture group were higher than that of in the femoral neck fracture group(P 〈0.001, respectively). Conclusion Study of senile hip fracture of two types reveals that there are differences in expressions of local IGFBP-4, which nevertheless have different regulatory mechanisms from those of peripheral circulation expressions except for IGFBP--4. Because of the complexity of IGF-1 and IGFBP regulatory mechanisms, a large number of experimental data are still in need as evidence to further reveal their relationship with fracture types.
