癌症放疗患者不同心理干预方式效果研究

摘　　要：目的:探讨音乐放松训练和抗抑郁剂对癌症患者在放射治疗期间抑郁、焦虑情绪的影响。方法:92名接受放射线治疗的癌症住院患者按照接受放射治疗的时间先后分为三组,其中一组30名患者在放疗过程中接受音乐放松训练的心理干预,一组31名患者接受抗抑郁剂盐酸氟西汀干预,另一组31名患者不干预(对照组)。且在放疗的不同时期采用汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(HAMD)进行测量。结果:音乐放松训练组比对照组患者的焦虑明显减轻(P<0.05),抑郁情绪减轻无统计学意义(P>0.05);抗抑郁剂药物干预组比对照组患者的焦虑情绪、抑郁情绪都明显减轻(P<0.01);与放疗开始第0周相比,放疗开始后第4周与放疗开始后第8周的焦虑、抑郁情绪明显减轻(P<0.01);与放疗开始后第4周相比,放疗开始后第8周时的焦虑、抑郁情绪反弹(P<0.01)。结论:在临床上对于癌症放疗患者给予抗抑郁剂药物的干预和音乐放松训练心理干预是有必要的,对缓解或减轻患者焦虑、抑郁等情绪有一定的帮助,从而增加其癌症治疗的依从性。Objective： To investigate the effects of training in relaxation with music and antidepressant medications on depression and anxiety in cancer patients at different radiation therapy stages.Methods： 92 hospitalized patients with cancer were divided into three groups by the order of time when they began to accept the radiotherapy.One group of 30 patients received training in relaxation with music,one group of 31 patients received antidepressant fluoxetine and the other group of 31 patients（control group） didn＇t.All patients filled out Hamilton Anxiety Scale（HAMA） and Hamilton Depression Scale（HAMD） at different radiotherapy stages.Results： Anxiety in the training in relaxation with music group was significantly lower than that in the control group（P0.01）,but it didn＇t work for depression;Both anxiety and depression in the antidepressant fluoxetine group were significantly lower than those in the control group（P0.01）;Compared with anxiety and depression at week 4 and week 8,anxiety and depression were significantly lower than those at week 0（P0.01）;but both were rebound significantly at week 8 than those at week 4（P0.01）.Conclusion： Both the clinical training in relaxation with music and antidepressant medications are necessary for that they can help the cancer patients undergoing radiotherapy to relieve soon and can increase the patients＇ compliance with the cancer treatment.
