化浊解毒消溃煎治疗消化性溃疡的临床研究

摘　　要：目的:观察化浊解毒消溃煎治疗消化性溃疡(peptic ulcer,PU)浊毒内蕴证的临床疗效。方法:将符合纳入标准的病例80例,随机分为治疗组和对照组,治疗组服用化浊解毒消溃煎,对照组服用盐酸雷尼替丁胶囊,两组均服药42 d。观察治疗前后主要症状改善情况及胃镜、幽门螺杆菌(helicobacter pylori,HP)变化情况。结果:①临床症状疗效比较:治疗后治疗组临床症状有效率87.5%,对照组75.0%,两组有效率比较,治疗组优于对照组(P<0.05)。②治疗前后主要症状积分比较:治疗组和对照组各症状均比治疗前有改善(P<0.05);治疗组与对照组症状改善比较,差异有统计学意义(P<0.05)。③胃镜疗效比较:治疗后治疗组胃镜有效率95%,对照组胃镜有效率72.5%;两组胃镜有效率比较,差异有统计学意义(P<0.05),治疗组优于对照组。④对HP影响的比较:治疗组治疗前HP阳性31例,治疗后HP阳性8例,清除率74.2%;对照组治疗前HP阳性30例,治疗后HP阳性16例,清除率46.7%;两组清除率比较,差异有统计学意义(P<0.05)。⑤复发率比较:治疗组复发率为9.5%,对照组为38.9%,两组复发率比较,差异有统计学意义(P<0.05)。⑥安全性评估:治疗前后两组患者血、尿、便常规,心电图、肝肾功能均无明显变化,提示两组药物均无明显毒副作用。结论:化浊解毒消溃煎能改善消化性溃疡浊毒内蕴证的临床症状,抑刹HP,提高溃疡愈合质量,降低复发率,其疗效作用优于雷尼替丁胶囊组。Objective： To observe the clinical effect of huazhuojieduxiaokui Fried treat on Peptic ulcer.Methods： Using randomized method to divided 80 cases into treatment group and control group randomly,the treatment group and control group each had 40 cases,the treatment group received huazhuojieduxiaokui Fried,the control group received ranitidine hydrochloride,six weeks for both groups.Observed Main symptoms improved and gastroscope,h.pylori（Helicobacter pylori,Hp） change condition before and after treatment.Results：①The comparison of clinical symptoms curative effect： The total effective of treatment group and control group was 87.5%,and 75.0 %.The treatment group was better than control group（P 0.05）.②The comparison of main symptom scores before and after treatment： 6 weeks after treatment,the treatment group and control group were improved than before treatment（P 0.05）,the treatment group showed a significant difference with control group in symptoms improve aspects（P 0.05）.③Gastroscope curative effect comparison： The total effectiveness of treatment group and control group was 95%,and 72.5%.There had a significant difference between the two groups（P0.05）,the treatment group than in control group.④Compared to Hp effect： The treatment group had 31 cases is positive,and 8 cases after treatment,its clearance rate was 74.2%;The control group had 30 cases before treatment,and 16 cases after treatment,its clearance rate was 46.7%.Two groups of comparisons has significant difference（P0.05）.⑤The long-term outcome comparison： The treatment group was 9.5% for recurrence rate,the control group was 38.9%,there had significant difference（P0.05）.⑥Safety assessment： For two groups of patients,routine blood and urine,electrocardiogram,liver and kidney function were not significantly change before and after the treatment,suggested that the two groups of drug had no obvious side effects.Conclusion： Clinical studies showed that huazhuojieduxiaokui Fried can improve the clinic
