脉络宁硬膜外注射配合手法复位治疗腰椎间盘突出手术后复发的临床研究

摘　　要：目的:探讨硬膜外注射配合手法复位对腰椎间盘突出手术后复发病例的疗效。方法:将70例腰椎间盘突出手术后复发患者随机平均分为硬膜外注射组(治疗组)和常规治疗组(常规组),疗程21-28 d,治疗前及治疗后1d、7 d和疗程结束后应用目测类比评分法(VAS)对疼痛进行评估,对症状、体征进行临床评定。结果:治疗组和常规组有效率、优良率分别为97.14%、71.43%和77.14%、42.86%,差异均有显著性意义(P<0.05)。常规组和治疗组治疗前VAS评分分别为(8.70±1.23)分和(8.80±1.28)分,常规治疗后第1天、第7天及第21-28天VAS评分常规组分别为(8.30±0.63)、(6.30±0.89)和(4.50±0.45)分,硬膜外注射组分别为(5.90±0.81)、(3.80±0.95)和(1.10±0.34)分,两组止痛效果比较,差异有统计学意义(P<0.05)。结论:硬膜外注射配合手法复位对腰椎间盘突出手术后复发病例有较好的临床疗效。Objective： To study the therapeutic effect of peridural injection and manual reduction on recurrent lumber disc herniation underwent operation.Methods： Between 2005 and 2010,70 patients（average age 44,range 27-61 years） with recurrent lumbar disc herniation underwent operation were randomly divided into the peridural injection group（n=35） and the traditional treatment group（n=35）.The clinical symptoms and signs as well as the scores of VAS were assessed pre-and post-treatment.Results： The rates of patients with improvement and excellence were 97.14 %,71.43 % and 77.14%,42.86% in the peridural injection group and the traditional treatment group respectively.There were significant differences between the two groups（P0.05）.Before treatment,the VAS score was（8.6±1.4）and（8.8±1.28）points in the traditional treatment group and the peridural injection group respectively,and,at 1 day,1 week and 3-4 weeks after treatment,it was（8.3±0.63）,（6.3±0.89）and（4.5±0.45） points in the traditional treatment group,and（5.9±0.81）,（3.8±0.95）and（1.1±0.34） points in the peridural injection group respectively.There was significant difference in analgesic effects between the two groups（P0.05）.Conclusion： Peridural injection combined with manual reduction is an effective therapeutic option for the treatment of recurrent lumber disc herniation.
