复杂髋臼骨折的手术治疗

摘　　要：目的探讨复杂髋臼骨折的手术方法和疗效。方法回顾分析2007年4月-2010年10月治疗的41例复杂髋臼骨折患者中获得随访的33例患者的手术治疗效果。按Letoumel分类方法,其中后柱加后壁12例,横行加后壁8例,T型骨折6例,前柱加后半横行4例,双柱骨折3例。采用不同手术入路,使用重建钢板及螺钉内固定治疗。结果获得随访的患者,平均随访时间12个月（6-36个月）。随访内容包括骨盆拍片和髋关节功能评分,按改良Merled’Aubigne-Postel标准评分,其中优20例,良7例,可6例,优良率达81.8%。结论通过正确判断骨折类型,选择合适的手术入路,良好的复位和可靠的内固定,复杂髋臼骨折可以获得满意的疗效。Objective To study the operative method and operative results for complex acetabular fractures.Methods From Apr2007 to Oct 2010,33 patients with complex acetabular fractures were treated by open reduction and internal reconstructive plate fixation.There were 20 males and 13 females,with a mean age of 39.4 years（range from 19 to 55 years）.According to Letournel classification of acetabular fracture,Kocher-Langenbeck approach,ilioinguinal approach or Stoppa approach and anterior combined posterior approach were adopted for different fractures.A complete physical examination with use of the modified Merle d＇Aubign€？score was preformed to evaluate the results of acetabular fracture treatment.Results All patients were followed up for an mean duration of 12 months（range from 6 to 36 months）.Fracture union was obtained in all the patients.Base on the modified Merle d＇Aubign€？score,the result was excellent in 20 patients,good in 7,fair in 6,with the excellent-good rate of 81.8%.Conclusion Correct classification of the acetabular fracture,choose appropriate surgical approach,good reduction and stable internal fixation were the essence in the surgical treatment of complex acetabular fracture.
