摘 要:目的:研究绝经后子宫出血的原因和最佳诊断方法。方法:对1994年9月~1999年2月我院收治的435例绝经后子宫出血患者进行B超检查和宫腔镜检查加刮宫或组织活检。结果:单纯子宫病变占972%,单纯附件肿瘤或附件肿瘤与宫内病变并存占28%;子宫与附件的良性病变占906%,恶性肿瘤占94%;宫腔镜下诊断宫腔内良性非赘生性病变与病理的总符合率为100%,诊断赘生性病变与病理的总符合率为968%。结论:绝经后子宫子宫出血的原因以子宫的良性病变占绝大多数;随年龄增长,恶性肿瘤的发生率有上升趋势;宫腔镜检查对绝经后子宫出血的诊断具有很高的价值。Objective: To investigate the causes of postmenopausal uterine bleeding and the best diagnostic methods. Methods: Ultrasonography and hysteroscopy with dilatation and curettage(or biopsy) were performed from september 1994 to February 1999 on 435 postmenopausal women suffering from uterine bleeding. Results: Uterus disease alone was found in 97.2% of patients, overian and fallopian tube tumor or overian and fallopian tube tumor with uterus disease were found in 2.8% of patients. The correspondence rate of hysteroscopy to histopathological findings was 96.8% to 100%. Conclusions: Benign lesions had been detected in most of the patients with postmenopausal uterine bleeding. The probability of cancer as the underlying cause increased with age. Histeroscopy is of great value in detecting the causes of postmenopausal bleeding, combinatioh of hysteroscopy and ultrasonography is the best method in diagnosing postmenopausal bleeding.
