摘　　要：目的评价枕颈部减压＋Cervifix系统内固定融合的手术方式治疗Arnold—ChiariI型畸形的临床治疗效果。方法回顾分析我科自2005年4月至2010年4月用枕颈部减压＋Cerv—ifix内固定方法治疗ChiariI型畸形患者12例，JOA评分比较术前术后及随访1年后的临床神经症状恢复情况，评价术后的改善的优良率及短期随访的临床效果。结果术后及随访1年的患者的临床症状均有明显的改善，JOA评分从术前平均7．7分提高到术后的12．9分，改善的优良率为83．3％。1年的随访JOA平均分为13．4分。结论枕颈部减压＋Cervifix内固定融合手术是治疗ChiariI型畸形的有效方法。Objective To evaluate the clinical efficacy of occipitocervical decompression and cer- vifix system internal fixation on Arnold - Chiari I malformation. Methods The 12 Arnold - Chiari Ⅰ cases in our department from April 2005 to April 2010 were treated with occipitocervical decompression and Cervifix system internal fixation. Clinical nervous function recovery was analyzed retrospectively through JOA score before and after operation, and during a follow - up period of 1 year. Meanwhile, the excellent and good ratios after operation, and the clinical effectiveness during the follow -up period were evaluated. Results Good function recovery was found after operation and during one - year follow - up period. JOA score was elevated from pre - operative day 7.7 to post - operative day 12.9. Excellent and good ratios were 83.3%. Average JOA score was 13.4 after 1 - year follow - up. Conclusion Occipitocervical decompression and Cervifix internal fixation should be effective methods to treat the Arnold - Chiari Ⅰ malformation.
