荧光原位杂交与免疫组化检测结直肠癌组织HER-2基因扩增或蛋白表达情况的研究

摘　　要：目的:用免疫组化(IHC)和荧光原位杂交技术(FISH)检测结直肠癌组织中HER-2蛋白表达或基因扩增状况,探讨HER-2在结直肠癌中的表达及临床意义。方法:采用IHC EnVision法检测68例结直肠癌及40例癌旁粘膜组织中HER-2蛋白的表达情况,并分析其与结直肠癌临床病理特征和预后的关系;对IHC(++)和(+++)的15例再进行FISH检测HER-2基因扩增状况。结果:HER-2蛋白在结直肠癌及癌旁粘膜组织中的过表达率分别为22.1%和2.5%,差异有统计学意义(P<0.01);在结直肠癌中,HER-2蛋白的过表达与发病年龄、民族有关(P<0.05)。对于IHC(+++)的病例,均可检测出HER-2基因扩增,IHC(++)的病例HER-2基因扩增率为23.1%(3/13)。结论:HER-2可作为评价结直肠癌发生、发展的生物学指标,HER-2蛋白表达(++)的病例有必要进一步进行HER-2基因扩增检测,来确定是否能用针对HER-2的靶向治疗。Objective：To detect HER-2 expression in colorectal cancer tissues using immunohistochemistry（IHC） and fluorescence in situ hybridization（FISH）,to analyze the significance of HER-2 expression in colorectal cancer. Methods：The expression of HER-2 protein in 68 cases of colorectal cancer and 40 cases of peri-cancer mucosa was detected by EnVision IHC.The correlations of HER-2 expression with clinicopathological parameters and prognosis in colorectal cancer were analyzed.15 cases of positive（＋＋） and（＋＋＋） by IHC and then detected the amplification of HER-2 gene by FISH. Results：The over-expression rates of HER-2 in colorectal cancer and peri-cancer mucosa were 22.1% and 2.5%,there is statistic significance（P〈0.01）,in colorectal cancer,the strong expression of HER-2 was colosely related to the age and race（P〈0.05）.Cases of positive（＋＋＋） by IHC could all detected HER-2 gene amplification,of positive（＋＋） cases could detected 23.1%（3/13） HER-2 gene amplification. Conclusion：HER-2 could be used as biological factors of evaluating genesis and progression of colorectal cancer.In the cases with positive（＋＋） expressions of HER-2,the amplifications of HER-2 gene should be further tested to guide the targeted therapy in the treatment of colorectal cancer.
